蛛网膜下腔注入艾芬地尔对大鼠炎症痛阈的影响

摘  要:目的研究N-甲基-D天门冬氨酸(NMDA)2B亚型受体特异性拮抗剂艾芬地尔对大鼠炎症两个时相痛阈的影响。方法分别对大鼠进行蛛网膜下腔置管,1周后,取无活动和感觉障碍大鼠随机分为4组,为正常对照(N)组、模型(M)组、氯胺酮(K)组、艾芬地尔(I)组。N组大鼠右侧后脚掌注入生理盐水100μl,其它各组分别右侧后脚掌注入相同容量2%角叉菜胶(carrageenan)致炎。各组在致炎后3 h,蛛网膜下腔注射相应容量药物,N组和M组大鼠分别注入生理盐水,致炎后3、15h观察其机械痛阈值和热辐射潜伏期。结果M组两个时相上痛阈值明显降低;氯胺酮在早期时相镇痛作用较弱,而在第二时相上镇痛作用较强(P<0.05);艾芬地尔在两个时相都能够产生较强镇痛作用(P<0.05);这两种药物都有提高正常大鼠痛阈值作用(P<0.05)。结论氯胺酮、艾芬地尔都有镇痛作用,但是其起效时间及作用机制可能不同。对炎症性疼痛,蛛网膜下腔用ifenprodil可以有比氯胺酮更好的镇痛效果。Objective To evaluate the effects of N-Molhyl-d-Aspartate(NMDA) receptors subunit NR2B antagonist(ifenprodil) administrated subarachnoidly at two phases of inflammatory pain. Methods The rats implanted with chroinc lumbar intrathecal catheters were divided randomly into control(N), model(M), ketamine (K) and ifenprodil (I) groups. The rats in group N received normal saline in the right hind paw,but the other groups received 2% carrageenan 100 μl. Three hours after injection of carrageenan the drugs were injected in group K and I, and normal saline was iniected into ~he subarachnoid space in group N and M. The mechanical threshold of pain and the thermal latency were observed at 3 and 15 hours after inflammation occureence. Results From the model groups, the mechanical threshold and thermal latency at two phases were reduced significantly in group M more than those in group N, K and I(P〈0.05). Ketamine could attenuate pain at the late phase(P〈0. 05), but the ifenprodil did it at two phases(P〈0. 05). Condusion Both ketamine and ifenprodil could elevate pain threshold of the inflammation with different mechanism and latency. Intrathecal ifenprodil has better analgesic effect than ketamine.
