孕母间断补铁对预防婴儿缺铁性贫血的作用观察

摘　　要：[目的]观察孕母间断补铁预防新生儿及婴儿缺铁性贫血(iron deficiency anemia,IDA)的作用。[方法]对预防组109例孕妇从孕20周开始进行两周一次的间断补铁,每次补充元素铁105 mg,共8次,对照组109例不补铁剂。实验前后对两组孕妇血以及脐血检测血红蛋白质(Hb)、血清铁蛋白(SF)、红细胞锌原卟啉(ZPP)等指标。对两组婴儿随访6-12月,评估婴儿IDA的患病情况。[结果]实验前两组孕妇问Hb、SF、ZPP差异无显著性(P>0.05);实验结束后对照组各项指标均低于预防组,差异有非常显著性(P<0．01);预防组新生儿脐血SF值和出生体重高于对照组(P <0．05);预防组婴儿6月龄时Hb均数高于对照组(P<0．05)。[结论]孕期间断补铁可明显改善孕妇及胎儿或新生儿铁营养状况,有利于减少婴儿IDA的发生。[Objective] To survey the effect of providing the pregnant with supplement of iron intermittently to affect the iron deficieny anemia in the period of newborn and infant. [Methods] From 218 pregnant women ,the studied group and the contrasted group were divided randomly, with 109 for each group. The iron supplment was given to the studied group beginning at the 20th week of the pregnancy, and without the supplement for the contrasted group. The follow-up was made at the pre/after the iron supplement to survey the every index about iron nutritional comdition of the pregnant women,the newborns and the infants . The index included the HB,SF,and ZPP. [Results] Between the two groups of the pregnant women at the 20th week of the pregnancy,the difference of the HB,SF, ZPP could not be seen （P〉0.05）. Evcry indexs about iron nutritional condition of the contrasted group were all lower than those of the studied group after the study finished,and the difference was very remarkable （P〈0.01）. The level of SF in the umbilical blood as well as the weight of the newborn in the studied group was much higher than that of the contrasted group （P〈0.05）. The average concentration of HB at 6 months in the studied group was higher than that of the contracted group. [Conclusions] Given supplement of iron intermittently in the period of the pregnancy can improve the iron nutrition for the pregnant women,fetus and infants effectively,and it is helpful to reduce the incidence of the iron deficiency anemia of the infants.
