摘  要:[目的]研究不同剂量国产促红细胞生成素(rhuEPO)防治早产儿贫血的疗效及副作用。[方法]将符合研究条件的56例早产儿随机分为三组,对照组和大剂量组、小剂量组。大、小剂量组于生后第8 d给予rhuEPO治疗,剂量分别为500 U/(kg·周)和250 U/(kg·周),每周分3次皮下注射,共6周。三组均于治疗一周后予口服元素铁6 mg/(kg·d)。[结果]三组早产儿生后血色素和红细胞压积均逐渐下降,大剂量组下降幅度最小,对照组下降最明显, 三组差异均有显著性;大、小剂量组于治疗一周后网织红细胞即明显上升,于第4、5、6、7、8周与对照组比较差异有显著性 (P<0.05、0.01);三组血清铁蛋白生后均呈下降趋势,下降幅度三组比较差异有显著性,于生后第8周(停药后1周)三组差异无显著性;对照组与小剂量组输血例数比较,差异无显著性(P>0.05);大剂量组输血例数较小剂量组和对照组减少, 差异有显著性(P<0.05)。两用药组无不良反应发生。[结论]国产rhuEPO辅以铁剂可有效防治早产儿贫血,减少输血次数,且500U/(kg·周)效果优于250 U/(kg·周)。[Objective] To study the efficiency and side effect of different doses of the internal rhuEPO in preventing and treating anemia of preterm infants. [Methods] Fifty-six preterm infants fitting for the demands were divided into three groups at random, the contract group, the high-dose group, and the low-dose group. The high-dose group and the low-dose group were given rhuEPO 500 U/(kg·w) and 250 U/(kg·w), respectively, which were devided into three times a week for subcutaneous injection, total for six weeks, from eight day after birth. And the three groups were all given oral iron therapy 6 mg/(kg·d). [Results] Hemogolobin and hematocrit declined in the three groups. The decline range of the high-dose group was minimum, while that of the contract group was maximumt There were significant difference among the three groups(P〈0.01 or 〈0.05). Reticulocyte increased obviously after therapy for one week in high-dose and low dose groups of which Ret had significant difference from the contract group in 4^th, 5^th , 6^th ,7^th and 8^th week, respectively. And the frequency of transfusion decreased significantly in high-dose group. No side effect happened in all groups. [Conclusion] The internal rhu-EPO with oral iron therapy can prevent and treat anemia of preterm infants effectively, and decrease freqence of transfusion. Moreover, effect of 500 U/(kg·w) is better than that of 250 U/(kg·w).
