摘 要:目的探讨肝肿瘤行肝动脉栓塞术后肿瘤转移的发生及其分子生物学机制。方法30只Wistar大白鼠种植Walker 256后第8天随机分为两组,行肝动脉结扎(HAL)和单纯开腹手术。术后第1、3、7天各处死三只大鼠,各组剩下6只大鼠于术后第12天处死,切取肿瘤及双肺,采用肉眼及显微镜评估肺转移情况;通过免疫组织化学染色检测不同时间点的缺氧诱导因子-1α(HIF-1α)及血管内皮生长因子(VEGF)表达。结果HAL术后第1、3天,肝动脉结扎组的HIF-1α及VEGF表达显著高于剖腹对照组。至术后12天,HAL组共3只大鼠观察发现肺转移病灶,而剖腹对照组仅1只发现肺转移结节。结论肝动脉结扎术后肿瘤组织缺血缺氧加重,HIF-1α与VEGF蛋白表达增加,可诱发肺转移。Objective To investigate the effect of hepatic arterial blockage on metastasis of hepatic tumor and the feasible molecular biologic mechanism. Methods Thirty Wistar rats were randomly divided into two groups after Walker 256 carcinoma was implanted in the left lobes of the livers. Hepatic artery ligation (HAL) or abdominal incision was performed. Each three rats were sacrificed 1, 3, 7 d and the remain six rats were sacrificed 12 d after treatment. Tumor tissues and lungs were excised. Pulmonary metastases were evaluated using macroscopic and microscopic method; Expression of hypoxia-inducible factor-1α (HIF-1α) and vascular endothelial growth factor (VEGF) protein of tumors in different time were examined with immunohistochemistry. Results 1, 3 d after treatment, the staining intensity of HIF-1α and VEGF was significantly higher in the HAL group compared with the control group. Till 12 d after treatment, pulmonary metastatic lesions were observed in three rats in the HAL group in comparison with one rat in the control group. Conclusion HAL aggravates hypoxia in tumor, and promotes the expression of HIF-1α and VEGF protein, which may stimulates pulmonary metastasis.
