氯氮平和思瑞康对精神分裂症患者血糖代谢的影响

摘　　要：目的探讨服用抗精神病药物氯氮平和思瑞康对糖耐量的影响。方法92例精神分裂症患者随机分为氯氮平组和思瑞康组,各46例,两组患者治疗前和治疗后第4周末、8周末分别检测空腹血糖、餐后1h、2h和3h血糖,并分析糖耐量。结果治疗后4周末和8周末,氯氮平组餐后1h和2h血糖浓度与治疗前比较明显升高[(8.6±1.6)(8.8±1.8)mmol/Lvs.(7.1±1.1)mmol/L,(7.2±1.1)(7.4±1.2)mmol/Lvs.(5.8±0.8)];而思瑞康组无明显变化。治疗后氯氮平组(21.7%,10例)患者糖耐量减低的发生率高于思瑞康组(4.3%,2例),差异有显著性(χ2=6.13,P<0.05)。结论氯氮平对精神分裂症患者餐后血糖有影响,而思瑞康对血糖的影响甚微。Objective To investigate the effects of clozapine and Seroqual on glucose metabolism in treatment of schizophrenic patients. Methods 92 schizophrenics were randomly divided into two groups, each group included 46 and was treated with Clozapine and Seroqual respectively. Before and after 8-week treatment, all patients were given glucose tolerance test and glucose test respectively. Results The blood glucose at the time-peints of land 1 hour and 2 hours were higher than the baseline in clozapine group, but unchanged in Seroqual group. The rate of impaired glucose tolerance was higher in clozapine group（21.7%, 10 case）than that in Seroqual group （4.3%, 2 case）（X^2=6.13, P〈0.05）. Conclusion Clozapine has stronger effect than Seroqual on the glucose metabolism.
