耐亚胺培南铜绿假单胞菌耐药性分析

摘  要:目的:探讨住院病人临床标本中分离的耐亚胺培南铜绿假单胞菌(IRPA)的耐药情况,以指导临床有效使用抗生素。方法:按常规方法对铜绿假单胞菌进行分离培养,VITEK—Ⅱ全自动细菌鉴定仪进行药敏实验采用纸片扩散(K-B)法,按2005年NCCLS标准对结果进行判读,使用whonet5.3软件对数据进行统计处理。结果:60株IR-PA对14种常用抗生素的耐药率分别为:头孢他啶100.0%,头孢哌酮/舒巴坦98.0%,头孢吡肟72.0%,哌拉西林70%,哌拉西林/他巴坐坦71.7%,替卡西林/克拉维酸85.0%,美洛培南75%,氨曲南78.3%,环丙沙星51.7%,左氧沙星氟78.3%,阿米卡星35.0%,庆大霉素45.0%,复方新诺明98.3%。除环丙沙星、庆大霉素、复方新诺明外,IRPA组的耐药率明显高于亚胺培南敏感铜绿假单胞菌(ISPA)组,差异有统计学意义(P<0.05)。结论:IRPA对大多数常用抗生素呈现出更强的耐药性。OBJECTIVE: Studying the resistance of [RPA (imipenem resistant Pseudomonas aeruginosa) isolated from the Second Affiliated Hospital SUN YAT - SEN University to direct the use of antibiotic agents. METHOD: Strains were isolated and cultured in routine procedures, and identified by VITEK - Ⅱ. Antibiotic Susceptibility Test (AST) were proceeded by disc - diffusion ( K - B ) method, the results were determined according to guidelines made by NCCLS ( National Committee Clinical Laboratory Standards). And whonet.5. 3 software was applied to dealing with the data. RESULTS: Resistant rate of IRPA tO 16 common used antibiotic agents are showed as follows : Ceftazidime ( 100. 0% ), Cefoperazone/sulbactam ( 98. 0% ), Cefepime ( 72. 0% ), Piperacillin (70%), PiperacilIin. tazobactam (71. 7% ), Ticarcillin - clavulanic acid (85%), Meropenem (75%), Aztreonam (78. 3%), Ciprofloxacin (5 1. 7%), Levofloxacin (78. 3%), Amikacin (3 5%), Gentamitin (45. 0% ), Trimethoprim - sulfonamides ( 98. 3% ). Except, Gentamicin, Ciprofloxacin and Trimethoprim - sulfonamides, resistant rate of IRPA group is obviously higher than the ISPA group. And the difference iS significant ( P 〈 0. 05 ). CONCLUSIONS: IRPA demonstrate higher resistance to common used antibiotic agents.
