治疗鼻出血鼻腔填塞物的选择

摘　　要：目的 为了比较鼻出血治疗中使用凡士林纱条、艾氟隆止血海绵和瑞纳凝胶快速止血材料的疗效及术后局部及全身反应情况。方法对150例鼻出血忠者分别采用凡士林纱条、艾氟隆止血海绵和瑞纳凝胶快速止血材料填塞鼻腔，24-48小时取出填塞物，根据填塞后出血情况评价疗效，根据局部疼痛、填塞物腐臭程度和体温变化评价局部及全身反应状况。结果凡士林纱条组的治愈率为62．0％，总有效率为90．0％；艾氟隆组的治愈率为68．0％，总有效率为84．0％；瑞纳组的治愈率为88．0％，总有效率为94．0％。瑞纳组治愈率较凡士林油纱条组治愈率和艾氟隆组治愈率有显著性差异（P=0．010，X^2=9．269），而三者总有效率无显著差异（P=0．267，X^2=2．641）。凡士林纱条组的局部和全身反应轻度的比率为28．0％；艾氟隆组轻度的比率为32．0％；瑞纳组轻度的比率为56．0％。瑞纳组轻度的比率较凡士林纱条组、艾氟隆组轻度的比率有显著性差异（P=0．008，X^2=9．606）。结论在鼻出血的治疗中使用瑞纳凝胶快速止血材料效果可靠，局部及全身反应轻，有推广应用价值。Objective To compare the hemostasis efficiency of paraffin gauze, ivalon group, and rapid RHINO Gelknit. Study their response of local and whole body. Methods Total 150 eases with eplstaxis were randomly packed using three different nasal packing materials . In which, paraffin gauze group 50 eases, Ivalon group 50 cases, rapid RHINO Gelknit group 50 eases. The packing materials were removed after left for 24 hours to 48 hours. The efficacy of nasal packing materials was assessed in terms of levels of nasal bleeding after the nasal packing. Reaction of local and body was assessed in terms of the levels of nasal pain, smell of the packing materials and change of temperature. Results The cure rate was 62.0% in paraffin gauze group, 68.0% in Ivalon group, 88.0% in rapid RHINO Gel-knit group. The effective rate was 90.0% in paraffin gauze group, 84.0% in Ivalon group, 94.0% in rapid RHINO Gel-knit group. The significant statistical difference of the cure rate and effective rate were observed between rapid RHINO Gel-knit group and the other two groups（ P =0.010, X^2=9.269）. The incidence of gentle effect of local and body was 28.0% in paraffin gauze group, 32.0% in Ivalon group, 56.0% in rapid RHINO Gel-knit group. The statistical difference was also significant （ P = 0. 008, X^2 = 9. 606）. Conclusions Rapid RHINO Gel-knit does well in control bleeding in the treatmen of epistaxis, response of local and whole body is very gentle.
