乙型肝炎病毒C基因启动子双突变与肝硬化、肝癌的临床研究

摘　　要：目的:分析乙型肝炎病毒(HBV)C基因启动子(BCP)双突变与乙型肝炎肝硬化、肝癌的关系。方法:针对BCP双突变的特点,设计一条通用捕捉探针,一条野生型和一条双突变显色探针。待测标本DNA经聚合酶链反应(PCR)扩增后分别与野生型和双突变显色探针杂交,然后用酶联免疫吸附实验(ELISA)显示杂交结果,判定BCP突变与否。结果:87例经证实为HBV-DNA阳性的慢性肝炎、肝硬化、肝癌患者中共有46例双突变,其中38例有单纯双突变,8例为混合突变(双突变和野生型皆为阳性)。25例慢性肝炎,有5例单纯双突变,1例混合突变。32例肝硬化有20例单纯双突变,3例为混合突变。30例肝细胞癌有13例单纯双突变,4例混合突变。结论:BCP双突变可能与慢性肝病进展有关。Objective：To detect double-position mutation in the basic core. promoter（BCP） of hepatitis B virus, and explore its relation to hepatoeirrhosis caused by hepatitis B as well as liver cancer. Methods：87 patients identified positive HBV-DNA from two university hospitals were enrolled in this study, in 25 eases of whom with chronic hepatitis, 32 hepatocirrhosis, and 30 liver cancer, respectively. All of the patients met the criteria of prevention and treatment for viral hepatitis published by Chinese Medical Association in 2000. To detect BCP double-position mutation, one common capture probe, and one mutant-specific detector probe as well as one wild-type detector probe were synthesized and hybridized with amplified HBV DNA from the sera of the subjects. ELISA was employed to show whether there was a BCP mutant or not. Results：Out of 87 patients,46 confformed BCP double-position mutant,38 of whom were BCP single-position mutant, and 8 mix mutant （ both mutation and wild-type were positive）. In 25 patients with chronie hepatitis, 5 were detectedas single-position mutation and 1 mix mutation; in hepatocirrhosis,20 vs. 3;and in liver cancer. 13 vs.4, respectively.Conclusion： The occurrences of BCP double-position mutation are significantly higher in hepatocirrhosis and liver cancer than that in chronic hepatitis B. BCP double position mutation in the basic core promoter （BCP） of hepatitis B virus might be involved in the progress of chronic hepatopathy.
