摘 要:目的:探讨慢性乙型肝炎患者血清蛋白电泳与肝组织病理分级分期的关系。方法:用法国产sebiaHYDRASYS全自动电泳分析仪及其配套的血清蛋白电泳试剂盒检测236例慢性乙型肝炎患者血清中白蛋白(AIb):α1、α2、β、γ球蛋白;同时行肝组织活检并对肝组织进行炎症活动度分级(G)和纤维化程度分期(S)。结果:随着肝脏的炎症和纤维化程度分别加重,AIb、α2球蛋白逐渐下降,α1、γ球蛋白逐渐升高。AIb、α2、γ球蛋白在G2、G3、G4比较差异有显著性(P<0.05),α1球蛋白在G4与G1、G2,G3与G2比较差异有显著性(P<0.05);AIb、α1、γ球蛋白S1、S2与S3、S4比较差异有显著性(P<0.05);α2球蛋白在S2、S3、S4比较差异有显著性(P<0.05)。G3、G4、S3、S4级患者α2球蛋白降低,G4期患者γ球蛋白增高。结论:AIb、α1、α2、β、γ球蛋白能反映肝损害的严重程度和肝纤维化的程度,其中α2、γ球蛋白最有价值。Objective:To study the relationship between serum protein electrophoresis and grading and staging of liver tissue in patient with chronic hepatitis B. Methods:The serum protein electrophoresis of 236 patients with chronic hepatitis B was determined with sebia HYDRASYS autochemieal analyzer made in France. Liver biopsy was conducted simultaneously to determine the inflammation grade and fibrosis stage of the liver tissues. Results:The hepatic activity index and .hepatic fibrosis increased as albumin and α2-globulin level decreased gradually,αl-globulin and γ-globulin level increased gradually. There was obviously difference of albumin,α2-globulin, γ-globulin level in G2, G3 and G4(P〈0.05).There was obviously difference of αl-globulin level in G4 and G1, G2,in G3 and G2(P〈 0.05). There was obviously difference of albumin, α1l-globulin, γ-globulin level in S1 ,S2 and S3, S4(P〈0.05). There was obviously α1- ference of α2-globulin level in S2 and S3 and S4 (P〈0.05). Conclusion:The serum protein electrophoresls could reflect the severity degree of grading and staging of liver tissues in patient with chronic hepatitis B . The α2-globulin and γ-globulin were useful index.
