彩色多普勒超声在肝胰十二指肠器官簇移植术后的应用价值

摘　　要：目的探讨彩色多普勒超声(CDUS)在肝胰十二指肠器官簇移植(combinedliver-pancreasen-bloctransplantation)中的应用价值。方法用彩色多普勒超声连续观测4例患者移植物的声像图改变和动静脉的血流变化,并与手术及临床结果对比。结果4例患者中,超声诊断术后并发症3例,均经手术证实。其中吻合口肠瘘的超声图像特征为全腹部大量气体,上腹部腹壁下较多游离性气体;移植胰脾静脉血栓形成图像特征为移植胰后方脾静脉内可见密集的光点回声,彩色及频谱多普勒脾静脉内未见明显彩色血流信号及静脉血流频谱;腹腔血肿超声表现为移植肝肝左叶后方与移植胰前方的不规则形无回声区。结论彩色多普勒超声在CIPBT中具有重要的应用价值。Objective To evaluate the value of color Doppler ultrasonography（CDUS） after the combined liver- pancreas en-bloc transplantation. Methods Ultrasound image features and changes of blood flow inside the transplanted organs were observed after combined liver-pancreas en-bloc transplantation in 4 patients via color Doppler ultrasonography and were compared with operations and clinical outcomes. Results On CDUS Three patients were diagnosed to have complications confirmed by operations and clinical outcomes. One patient had anastomotic stoma intestinal fistula found lots of abdominal gas, especially free under the abdominal wall on the sonogram. One patient became simple liver transplantation because of venous thrombosis of the transplanted pancreas. There was intensive light spot echoes in the splenic vein, but there was no color blood flow or venous spectrum in color Doppler flow imaging. Haematoma in the abdominal cavity was found as an irregular echo free zone between left hepatic lobe and the transplanted pancreas. Conclusions Color Doppler ultrasonography is very valuable to proviele informations for the combined liver-pancreas en-bloc transplantation.
