71例康复期SARS患者的胸部影像表现

摘　　要：目的:探讨严重急性呼吸综合征(SARS)康复期患者胸部影像学表现。方法:2003年6月至2005年9月,71例SARS康复期患者(27例为广医一院医护人员,25例为广州地区病人,19例为广东省其他地区病人)回院复查,根据病史及以往影像资料分为轻症和重症组,其中62例复查DR(数字化摄影)胸片141次,49例复查多层螺旋CT和高分辨CT73次,30例在同1 d作胸片、CT和肺功能检查。结果:62例DR胸片中完全正常的有27例,显著肺纹理聚拢和增强的6例,纤维索条影的21例,残留网格阴影的6例,胸膜增厚的2例。49例CT复查中,17例CT扫描未见异常,其余32例CT表现为(1)磨玻璃密度灶25例,包括弥漫分布和多叶多段病灶19例,局限性病灶6例;磨玻璃病灶伴随不规则纤维索条影15例,小叶间隔增厚22例,胸膜下线影7例,牵拉性细支气管扩张2例,胸膜小结节及小肺大泡各2例。(2)仅表现为局部索条影的7例。结论:部分SARS康复者肺部遗留间质炎症和纤维化,大部分病变随时间推移能逐步吸收。Objective：To study the imaging features in convalescent severe acute respiratory syndrome （SARS）. Methods： seventy-one patients with confirmed SARS, classified into 2 groups according to their history and previous imaging data, underwent follow-up digital radiography （DR） and/or HRCT of the chest and respiratory function tests. Results ： of 62 DRs, normal findings were shown in 27, aggregated or increased lung markings in 6, fibrous streaks in 21, grid shadows in 6 and pleural thickening in 2. Irrespective of 17 normal out of 49 CT scans, the other 32 exhibited： （1） ground glass opacities in 25 patients （19 diffuse or muhilobar, 6 localized）, including 15 complicated with irregular fibrous streaks, 22 with thickened intralobular septa, 7 with subpleural lines, 2 with traction bronchiolectasis, 2 with pleural nodules and 2 with minor blebs; （2） simple localized fibrous streaks in 7 patients. Conclusion： while residual abnormalities such as interstitial inflammation and fibrosis still remain in some discharge patients, the imaging features and respiratory function tests tend to be better over time.
