摘 要:目的了解老年人骨质疏松性骨折的风险与骨矿密度(BMD)的关系,为预测发生骨质疏松性骨折的风险提供科学的指标。方法对1020例年龄≥60岁的老年人骨质疏松症患者进行定量超声骨密度仪(QUS)测定跟骨的BMD值,将数据输入计算机进行统计学处理分析。结果骨折发生例数随BMD的T值降低呈负相关,骨质疏松性骨折患者百分比分别在Ⅰ区间和Ⅱ区间均与Ⅲ区有非常显著性差异,当BMD的T值≤-3SD骨折发生例数呈显著增加(r=-0.973,P<0.01)。结论老年人BMD的T值为-3.0SD时,骨折发生率呈明显上升,临床上可将老年人的BMD的T值低于正常1个标准差(-3.0SD)作为预测骨质疏松性骨折的风险指标。Objective To explore the relationship between the risks of the osteoporotic fracture in the elderly and the bone mineral density (BMD), and to provide the index for prediction of the risks of occurrence of osteoporotic fracture. Methods A population of 1 020 elderly patients with osteoporosis aged ≥960 years old underwent the examination of bone mineral density in the calcaneus by quantitative ultrasound (QUS) and the statistical analysis was performed with computer. Results The number of osteoporotie fracture had a negatively correlation to the BMD T value. There was significant difference in incidence of osteoporotic fractures between in - 2.0 SD≥T 〉 - 2.5 SD and - 2.5 SD≥T〉 - 3.0 SD and in T≤- 3.0 SD. When T≤ - 3.0 SD, the number of osteoporotic fracture was significantly increased (r=-0.973, P〈0.01). Conclusion When the T value of BMD in elderly is - 3.0SD, the occurrence of fracture is increased significantly. That the BMD T value is one standard deviation ( - 3.0 SD) lower than normal in the elderly can be used as the index of prediction of the risks of occurrence of osteoporotic fracture in clinics.
