同种异体血管移植制作动静脉内瘘时吻合口径不同对血液透析病人的临床研究

摘　　要：目的研究动静脉内瘘的不同口径对血液透析的影响与临床意义。方法用7-0无损伤丝线修补裂口和0号线结扎动脉小分支,然后将尸体血管放入肝素盐水中备用。将桡动脉与头静脉或贵要静脉吻合,肱动脉与肘正中静脉做U形吻合,异体血管移植时与动脉行端侧吻合。结果A组病人中,桡动脉与头静脉搭桥5例,与贵要静脉搭桥27例,肱动脉与肘正中静脉搭桥2例,其中行U形搭桥术5例。1年通畅A组94.2%,B组97.8%。结论采用异体血管移植制作动静脉内瘘,穿刺方便,止血容易,是作为生物支架理想材料,并发症少等优点。Objective To study the affection of varied diameter of artery - vein shunt for liemodialysis patient and its clinical significance. Methods The cephalic vein was anastomosed with basilica vein and brachial artery was anastommosed with median vein of ＂u＂. The allogenic graft was taken side- end anastomosis. Results The unblocked rate was 94.2% in group A and 97.8% after 1 year. There were 5 cases of radial a. and cephalic v., 27 cases of radial a. and basilica v., 2 cases brachial a. and median v. with 5 ＂u＂ graft. Conclusion The allogenic vestal graft for artery - vein shunt might be an ideal bio material with less complication, easy puncture.
