摘 要:目的:研究怡肾汤在改善肾功能和减轻肾小球硬化中的作用。方法:将72只SD大鼠随机取8只为正常对照组,其余分为模型组(n=32)及怡肾汤治疗组(n=32)。模型组和治疗组大鼠行5/6肾切除术。术后,模型组大鼠予以自由饮食、生理盐水灌胃1ml·100g-1·d-1;怡肾汤治疗组大鼠予以怡肾汤灌胃(0.3ml·100g-1·d-1),1次/d。于术后第5、6、7、8周4个时间点分别分批处死大鼠,用PAS染色观察大鼠残肾肾组织形态学改变,计算肾小球硬化指数(GSI);用免疫组织化学方法分别检测肾组织中Ⅳ型胶原(Col-Ⅳ)及基质金属蛋白酶-2(MMP-2)的表达。结果:(1)细胞形态学观察示:模型组中肾小球内细胞数目增多明显,基质增多,有新月体形成;系膜区增宽染色深,毛细血管开放不良甚至闭塞,肾小囊囊壁增厚。治疗组细胞增生明显轻于模型组,毛细血管开放较好,肾小管结构基本正常。治疗组和模型组大鼠肾小球呈不同程度硬化表现,治疗组GSI显著低于模型组(P<0.05)。(2)免疫组化发现:与正常组和治疗组比较,模型组肾组织中Ⅳ型胶原表达强烈,肾小球硬化严重;怡肾汤治疗组大鼠Ⅳ型胶原表达减少而MMP-2表达强于模型组。结论:Ⅳ型胶原在5/6肾切除大鼠肾组织中有较高的表达。怡肾汤可减轻肾组织中炎性细胞浸润和细胞增生,降低Ⅳ型胶原的表达、增加MMP-2的表达,延缓肾小球硬化进程和肾功能恶化的速度。Objective: To investigate the protective effects of Yishen Tang on renal function and its pathological changes. Methods:Seventy two rats were divided into normal control group( n =8),model group( n =32) and treated group with Yishen Tang( n = 32), then the each group was divided randomly into different time points of weeks 5, 6,7 and 8. The 5/6 nephrectomy was performed on rats of model group and treated group. The model arid normal group were fed with water and normal food, while the treated group were intragastricly fed with Yishen Rang (0.3 ml·100 g^-1·d^-1). The rats were respectively sacrificed at weeks 5, 6,7and 8. Pathological changes were observed by light microscopy and glomerulosclerosis index (GSI)was counted; Immunohistochemistry was used to examine the expression of collagen Ⅳ (Col-Ⅳ ) and matrix metalloproteinases 2(MMP - 2) in renal tissues. Results: According to light microscopy, there were more svere cell proliferation, extracelluar matrix accumulation and crescent formation in glomeruli of rats in model group than those in treated group. Glomerulosclerosis was observed in both model group and treated group, but glomerulosclerosis index (GSI) is much higher in model group than that in treated group, The expression of Col - Ⅳ in model group was stronger than that in treated group, while MMP- 2 expression in model group is weaker than that in treated group. Conclusion: Yishen "Fang could prevent the progression of glomerulosclerosis in rats with 5/6 nephrectomy. The action mechanism might be through its suppressive effect on expression of Col - Ⅳ and accelerating effect on expression of MMP - 2.
