心血管危险因素与肱踝脉搏波传导速度的关系

摘　　要：目的探讨大动脉弹性下降的心血管主要危险因素及多种危险因素并存时对大动脉弹性的影响。方法应用脉搏波速度自动测量系统测定肱踝脉搏波传播速度(BaPWV),依据健康人群动脉弹性功能参考标准把受试者分为BaPWV异常组和对照组;根据危险因素个数多少将BaPWV分为4个亚组(0-3亚组)。结果(1)BaP-WV异常组的年龄、总胆固醇(TC)、空腹血糖(FBG)、收缩压(SBP)和舒张压(DBP)、平均动脉压(MAP)、脉压(PP)明显高于对照组,而BaPWV明显高于对照组(P<0.05);单因素相关分析,年龄、SBP、DBP、MAP、PP、FBG、TC、HR分别与BaPWV呈正相关。多元逐步回归分析显示,年龄和收缩压是影响BaPWV的独立因素。(2)随着危险因素聚集程度的增加,0至3亚组BaPWV呈依次上升趋势(12.86±1.37、14.42±2.06、15.31±2.31、16.54±3.50,P<0.05)。结论年龄、SBP水平增加是大动脉弹性下降的主要心血管危险因素,多种危险因素并存时,可通过协同作用使大动脉弹性降低更明显。Objective To investigate the major cardiovascular risk factors affecting big arterial elasticity and the affect of combined milfiple risk factors on it. Methods Automatic pulse wave velocity （PWV） measuring system was applied to examine brachial - ankle PWV （BaPWV）. Subjects were categorized into abnormal BaPWV group and control group according to the level of BaPWV. The former group was further divided into four suhgroups（0to3） based on the mmber of risk factors. Results The level of sge, totle cholesterol （TC）, fasting blood glut cose（FBG）, Systolic blood pressure（SBP）, diastolic blood pressure（DBP）, Mean artery pressure（MAP）, Pulse pressure（PP）, Heart rate（HR） in abnormal BaPWV were higher than those in control group whereas BaPWV itself was higher than that in cintrol group（ P 〈 0.05）. Age, TC, FBG, SBP, DBP, MAP, PP were significandy correlated with BaPWV. A stepwise multiple regression analysis demomstrated that age and SBP were positively related to BaPWV independently. With the clusters of risk factors increasing BaPWV was increasing. Conclusion The elevation of age and SBP were the major cardiovascular risk factors on reduction of big arterial elasticity, with multiple risk factors concurrent effect, they could decrzase big arterial elasticity much mare significantly.
