高校学生270人的结核感染状况调查

摘　　要：目的 掌握近年来高校学生结核感染状况，了解其动态变化趋势，为结核病防治提供决策依据。方法在新生入校和第4学年时分别进行PPD试验和胸部X线检查。结果2001年调查入校新生270人，PPD试验阳性率为41．85％（113人），其中强阳性率为9．63％（26人）；无卡介苗预防注射疤痕95人（35．19％），其中无卡介苗预防注射疤痕而PPD阳性23人；城市籍新生结核感染率22．64％，农村籍新生结核感染率26．19％。对PPD试验强阳性者采取预防性治疗措施。2005年对该年级学生结核感染状况进行追踪调查，PPD试验阳性173人（64，07％），其中强阳性43人（15．93％）；无卡痕阳性47人；城市籍新生结核感染率67．92％，农村籍新生结核感染率26．19％。预防性治疗26人4年后PPD结果无变化，无新发结核病例。结论在该批学生中进行PPD试验调查，可以监测入校新生结核感染率，尽早筛出传染源，对PPD试验强阳性者可采取预防性治疗措施。Aim To evaluate the tubercular infection status in 270 undergraduate in university, to comprehend its changing trend, and to supply the evidence for prophylaxis strategy. Methods PPD test and chest X-ray examination were applied in students at their entrance and the 4^th year in the university. Results Among 270 freshmen in 2001, positive rate of PPD was 41.85% （113/270）, among which strong positive rate reached 9.63% （26/270）; and 95 students （35.19） were negative for PPD scar, among which 23 were strong positive for PPD test, TB infection rate were 22.64% in students from cities, and 26.19% in students from countryside. All students with strong positive result received prophylaxis treatment. All 270 students were checked again in 2005. Positive rate of PPD was 64.07% （173/270）, among which strong positive rate reached 15.93% （43/270）; and 47 students were negative for PPD scar. TB infection rate were 67.92% in students from cities, and 26.19% in students from countryside. Result of PPD test was not changed and no novel TB infection was encountered among 26 students received prophylaxis treatment. Conclusion the investigation could do help to monitor the TB infection rate in collage students, to detect source of infection. Prophylaxis treatment should be recommended in individuals with strong positive of PPD test.
