100例肝移植病例药物利用及费用分析

摘　　要：目的:分析肝移植患者住院期间药物利用与费用支出状况。方法:采用回顾性调查方法,提取2002- 2004年100例肝移植患者相关信息、用药医嘱与收费数据,并查阅网上电子病历。结果:执行医嘱37474条,涉及药品357种;维持水电解质平衡药、维生素与营养药、免疫系统、血液系统、消化系统、抗感染药物的出现频次占所有药品的80％;人血白蛋白、肝脏灌注液、肾离体保存液、他克莫司、人免疫球蛋白、头孢吡肟等在单品种出现频次与金额排序中居前位。药费、住院总费用及两者之比的均值分别为(9．15±3．70)万元、(21.57±6．64)万元、(41．57±5．23)％。影响费用的原因有术前机体状况、术后重症监护依赖、感染等。结论:该类患者的用药符合其特殊人群的定位。总住院费用明显低于文献报道,但药品费占总费用比例偏高。Objective： To analyze the drug utilization and cost in liver transplantations. Method： With a retrospective analytical method, 100 patients of liver transplantation who were admitted to the hospitals from 2002 to 2004 were recorded and their data concerning the involved information, prescriptions of drugs and their net data were reviewed. Resuit： 37 474 prescriptions were administered, involving 357 kinds of drugs. The frequency of appearance of several fundamental drugs accounted for 80 percent. They were the balance of liquid and electrolyte, vitamin and nutrition, immunity, hematic system, digestive system, antibiotics; human serum albumin, University of Wisconsin solution （UW）, hypertonic citrate adenine（ HC-A）, tacrolimus, human normal immunoglobulin, maxipime, et al occupied the front positions in the lists of individual frequency and their costs. The average drug cost and hospital cost were 91.5±37.0 and 215.7±66.4 thousand yuan, the proportion being 41.57%±5.23% between them. The preoperative physical state, the degree of ICU-bound postoperative and infection were risk factors for costs. Conclusion： The administration of drugs was in accordance with special individuals. The general cost of hospital was significantly lower than that of relative reports; however, the percentage of the drug expense of the overall medical expenses was higher.
