青少年女性特发性急迫性尿失禁盆底肌电刺激治疗近期疗效观察

摘　　要：目的 初步观察电刺激疗法治疗青少年女性特发性急迫性尿失禁（UUI）的近期疗效。方法 选取经临床和尿动力学检查证实为特发性急迫性尿失禁26例女性患儿年龄10～18岁，平均14±2岁）。使用神经肌肉电刺激治疗仪（NeuroTrac5^TM）治疗，每周3次，每次60min，疗程12周。治疗前后分别记录排尿日记、填写国际尿失禁咨询委员会尿失禁问卷简表（ICI-Q-SF）和进行尿动力学测定。结果 治疗12周后，11例（42．3％）尿失禁症状消失，12例（46．2％）尿失禁症状显著改善，3例（11．5％）症状无改善。症状消失和改善患儿治疗后72h总尿量、功能性膀胱容量、正常尿意膀胱压测定容量、最大膀胱压测定容量、最大尿道压和最大尿道闭合压显著高于治疗前（P〈0．05）；72h总排尿次数、72h总漏尿次数、尿失禁ICI-QSF评分和逼尿肌无抑制收缩压力和逼尿肌无抑制收缩持续时间显著低于治疗前（P〈0．05）。结论 应用电刺激疗法治疗青少年女性UUI近期疗效明显，具有疗效可靠、无创伤性和经济方便的优点。Objective To assess the effects of pelvic floor electrical stimulation in adolescents with idiopathic urgent incontinence using the NeuroTrac 5^TM. Methods Pelvic floor electrical stimulation was performed on 26 adolescent girls （age ranged from 10 to 18 years, average age 14 ± 2 years old） with idiopathic urgent incontinence for three months using the NeuroTrac 5^TM and skin surface electrodes. Urinary diary, International Continence Inquiring Committee＇s Questionnaire （ICI-Q-SF） scores were recorded and urodynamic study was performed before and after the treatment. Results The day urgent incontinence disappeared in 11 （42. 3%）, improved in 12 （46. 2%）, did not improved in 3 patients（11. 5%）. In the group with improved symptom, the total volume （72 h）, functional capacity, normal desired cystometric capacity, maximum cystometric capacity, max urethral pressure and max urethral closure pressure were significantly higher than those before treatment; the total time of voiding （72 h）, total time of leakage （72 h）, the scores of ICI-Q-SF, max detrusor uninhibited contraction pressure and detrusor uninhibited contraction duration were significantly lower than those before treatment （P〈0. 05）. There was not significant difference in max flow rate, bladder compliance, the detrusor pressure at max flow rate and static urethral functional length between before compared after treatment （P〈0. 05）. Conclusions Pelvic floor electrical stimulation treatment with NeuroTrac 5^TM. Is effective in the adolescents with idiopathic urgent incontinence. It is both convenient and economical.
