^(99m)Tc-ECDSPECT脑功能显像检测干燥综合征并发中枢神经系统损害异常rCBF29例临床分析

摘  要:目的探讨99mTc-ECDSPECT脑功能显像检测干燥综合征(SS)异常脑血流改变的临床价值。方法对19例SS伴发中枢损害症状或体征的患者进行99mTc-ECDSPECT脑功能显像,以10例SS无确切中枢损害症状或体征者作为对照组。结果19例SS伴发中枢损害患者99mTc-ECDSPECT脑功能显像检测出11例(57·9%)局部脑血流灌注(rCBF)减低,对照组仅1例(10·0%)rCBF减低,两组间差异有显著性意义(χ2=6·20,P<0·05)。最常被累及的区域为顶叶、颞叶。结论可以用99mTc-ECDSPECT脑功能显像检测SS伴发中枢损害患者的脑血流灌注减低。但该方法确切的灵敏度及特异性尚需进一步探讨。Objective To investigate the clinical value of Technetium - 99m - ethyl cysteinate dimmer ( ^99mTc- ECD) single photon emission computed tomography (SPECT) in detecting the changes of regional cerebral blood flow (rCBF) of patients with Sjogreng syndrome (SS). Methods ^99mTc- ECD brain SPECT was performed to detect the rCBF of 19 patients with SS complicated with brain lesions, and 10 patients with SS with no certain brain lesion as control. Results ^99mTc- ECD SPECT showed,, in 11 (57. 9% ) of the 19 trial patients, while in only one (10. 0% ) of the 10 patients, there was hypoperfusion of rCBF, with a significant difference ( P 〈0. 05), and the parietal and temporal lobes were the most common areas with such lesions. Conclusion This study suggests that 99aTe - ECD SPECT is an effective tool for detecting hypoperfusion of rCBF in patients with SS. However, the exact sensitivity and specificity of this method needs to be further explored.
