三氯乙烯药疹样皮炎与HLA-DMA及HLA-DMB的关系

摘　　要：目的根据人白细胞抗原(HLA)-DMA及HLA-DMB在三氯乙烯药疹样皮炎(DMLT)和正常对照分布情况,探索DMLT的易感性基因,为其危害防治提供依据。方法提取病例组(61例)及对照组(60例)淋巴细胞DNA,应用降落式多聚酶链反应(PCR)扩增,结合限制性片断长度多念性检测方法和直接测序,确定其DMA及DMB等位基因型和基因型,比较两组之间DMA和DMB等位基因型和基因型出现频率。结果病例组HLA-DMA*0101等位基因频率(71．3％)明显高于对照组(55．0％),差异有统计学意义(P<0．05);病例组HLA—DMB*0103等位基因频率(11．5％)明显高于对照组(3．3％),差异有统计学意义(P<0．05);对照组HLA-DMA*0102-*0102纯合子比例明显高于病例组,差异有统计学意义(P<0．05);病例组HLA-DMB*0101-*0102,杂台子比例明显低于对照组,差异有统计学意义(P<0．05)。结论DM基因多态性可能与DMLT的易感性有关。Objective To investigate the suscepubility of trichloroethylene-induced medicamentosa like dermatitis b） comparing the frequency of HLA-DMA and HLA-DMB in patients with trichloroethylene-indueed medicamentosa like dermatitis and in normal controls. Methods The DNA of lymphocytes in 61 patients with trichloroethylene-induced medicamentosa like dermatitis and in 60 people as the normal control were abstracted by using touchdown PCR amplification of HLA-DMA and HLA-DMB. Then thl＇ough restriction fragment length polymorphism （RFLP） and sequence base typing, the alleles and genotypes were confirmed. The frequency of HLA-DMA and HIA-DMB in the two groups was compared. Results The HLA-DMA ＊ 0101 allele frequency in patients with trichloroethylene-induced medicamentosa like dermatitis was significantly higher than in the control group（71.3% vs 55.0% , P 〈 0.05） .The allele frequency of HIA-DMA ＊ 0103 was significantly higher in the patient group than in the control group（11.5% vs 3.3% , P 〈 0.05） .The ratio of ＊ 0102 homozygotes of HLA-DMA ＊ 0102 in the patient group was significantly higher than in the control group（25.0% vs 8.2%, P 〈 0.05） .The ratio of ＊ 0102 heterozygotes of HLA-DMB ＊ 0101 in the patient group was lower than in the control group（P 〈 0.05）. Conclusion The polymorphisms of DMA may be related to the susceptibility of the patients with trichloroethylene-induced medicamentosa like dermatitis.
