保留直肠肌鞘的肛管吻合术后控便功能观察

摘　　要：目的探讨保留直肠肌鞘的低位肛管结肠/回肠吻合术后控制排便的功能。方法对13例直肠、结肠病变病人行上段直肠切除,距齿状线上1cm剥除下段直肠黏膜及黏膜下层,保留直肠肌鞘,行结肠/回肠与肛管吻合。结果13例病人近期内控制排便功能欠佳,但随着结直肠代偿功能的恢复,远期效果较满意。结论该方法术后效果满意,病人术后生存质量较高,易于接受。Objective To investigate the function of defecation after the anastomosis of coloanals or ileoanals reserved vagina of rectal muscul. Methods Thirteen patients were subjected to excision of upper part rectum or colon, aecollement of mucosa and submucosa layers in inferior rectum above anal, reserved vagina of rectul musculi and anastomosis of coloanals or ileoanals. Results The short-term effect of defecation controlling after surgery was not satisfactory in 13 cases. As time went on, this effect was satisfactory with the recovery of the function of compensation in colon and rectum. Conclusion The function of defecation and the quality of life were satisfactory after surgery. The anastomosis of coloanals or musculi may be an effective operation for restore the function of defecation.
