黑龙江省农垦地区肾综合征出血热Ⅰ型灭活疫苗免疫效果观察

摘　　要：为有效地预防肾综合征出血热（ＨＦＲＳ）的发生与流行，对沙鼠肾细胞型灭活疫苗的安全性及血清学、流行病学效果进行观察。结果：疫苗接种总反应率０．８９％（５５７／６２３１１），中反应率０．０３％（１９／６２３１１）；疫苗基础免疫全程接种两周后的血清中和抗体阳转率７０．３７％（３８／５４），荧光抗体阳转率８３．３３％（２５／３０）；垦区ＨＦＲＳ冬季主要宿主动物为黑线姬鼠，占６２．６１％（３６０／５７５），野外黑线姬鼠密度１１．１３％（２０４／１８３３），带病毒率６．０４９６（９／１４９），带病毒鼠指数０．０８２０；ＨＦＲＳ病人血清ＩｇＧ抗体检测为Ⅰ型，流行峰型为冬峰型，秋冬季节（８～１月份）发病人数占９３．９１％；健康人群隐性感染率３．４７％（５６／１６１６）。１９９５年１０～１２月份，疫苗全程接种组人群ＨＦＲＳ发病率０（０／１７１２７），全程非接种组人群发病率１０１．３６／１０万（１４４／１４２０６１），保护率１００％。由此显示：黑龙江垦区ＨＦＲＳ疫区类型为姬鼠型；该疫苗具有较高的安全性和较好的近期防病效果。Evaluation on the safety, immunological and preventive effects of inactivated HFRS vaccine produced from primary tissue culture of Mongolian gerbils kidney by Tianyuan Company, Hangzhou, was carried out in population in reclamation areas, Heilongjiang province from 1994 to 1995. The results showed that,the rate of total side effects of the vaccine was 0. 98%(557/62311),the rate of moderate and severe side effects was 0.03 % ( 19/62311).Two weeks after the third dose of immunization, the seroconversion rates were 70. 37%(38/54)by neutralization tests,83. 88%(25/30)by indirect immunofluorescent antibody tests.According to the epidemiological properties and serotyping results by IgG-ELISA of HFRS patient sera, the endemic area for evaluation was HTN type focus. For the evaluation, 17127 volunteers were vaccinated, 142061 nonvaccinated people lived in the same areas with the vaccinees were used for controL One year's observation showed that the protection rate was 100 %. All these showed that the vaccine was safe and effective.
