摘　　要：目的:寻找肝细胞癌微小转移的标志物。方法:用巢式逆转录聚合酶链反应(nestedRTPCR)技术检测60例肝细胞癌、20例非肝癌的恶性肿瘤、20例慢性乙型肝炎、20例肝硬化患者和20例健康志愿者外周静脉血中的glypican3mRNA。结果:Glypican3mRNA在健康志愿者、非肝癌恶性肿瘤、慢性乙型肝炎、肝硬化组患者的外周血中均为阴性;在肝细胞癌组外周血中glypican3的检出率为80%(48/60)。Glypican3检出率与临床分期、门静脉癌栓、肝外转移明显相关(P<0.05)。在12例血清AFP<25μg/L的肝癌患者血中,8例(66.7%)可检出glypican3。结论:Glypican3可作为血循环中有肝癌细胞的标志物,在肝癌患者中阳性预示有血源性转移的可能;并且glypican3对血清AFP阴性或低值的肝癌患者能起到辅助诊断作用。Objective：To identify the marker of hepatocellular carcinoma （HCC） micrometastasis. Methods.. Peripheral blood samples were obtained from 60 patients with hepatocellular carcinoma, 20 patients with other malignant tumors, 20 patients with chronic hepatitis B,20 patients with liver cirrhosis,and 20 normal healthy volunteers. Liver-specific glypican-3 mRNA expression was detected by nested reverse transcription-polymerase chain reaction （nested RT-PCR）. Results：Glypican-3 was not detected in the peripheral blood of normal healthy volunteers and the patients with other malignant tumors, chronic hepatitis B, and liver cirrhosis. The positive rate of glypican-3 mRNA in the patients with HCC was 80% （48/60）. It was correlated with the clinical stages of HCC, portal vein thrombosis,and extrahepatic metastasis. Glypican-3 was detected in 8 of 12 （66.7 %） HCC patients with low level of serum AFP（〈25 /μg/L）. Conclusion： The presence of glypican-3 in peripheral blood may be an indicator of malignant hepatocytes in peripheral blood. It predicts the possibility of blood metastasis of tumor cells in patients with HCC and plays an assistant role in the diagnosis of HCC in patients with negative or low level of serum AFP.
