摘  要:目的建立一种血清幽门螺杆菌(Helicobacterpylori,Hp)可溶性抗原检测方法,并评价其在Hp感染诊断中的作用。方法利用抗Hp单克隆抗体,采用双抗体夹心法检测血清中可溶性Hp抗原。并对其特异性和敏感性进行分析,同时与临床常用的血清特异性抗体检测、快速尿素酶试验、涂片镜检、14C尿素呼气试验和细菌培养5种方法进行了比较,并对药物根除效果进行了评价。结果血清可溶性Hp抗原检测方法敏感性和特异性分别为96.5%和96.0%,此方法明显优于血清特异性抗体检测、快速尿素酶试验、涂片镜检和细菌培养,与14C尿素呼气试验无显著差别,药物治疗前后血清Hp可溶性抗原含量有显著差异。结论血清可溶性Hp抗原检测方法是一种简便、高敏感性和特异性检测Hp感染的方法,可用于Hp感染药物根除效果的评价。Objective To establish a new serological method to detect serum soluble antigens of Helicobacter pylori (Hp) by monoclonal antibodies and its clinical application. Methods By using monoclonal antibodies, serum soluble antigens of Hp were detected by double antibody sandwich ELISA method and then compared with Hp serum specific antibody assay, rapid urease test, smear, ^14C- urea breath test and bacterial culture. Results The sensibility, specificity, positive predictive value and negative predictive value of the tested method were 96.5%, 96.0%, 98.2% and 92.3% respectively. After triple therapy, there was a significant difference of the concentration of Hp serum soluble antigens between the eradicated and not yet eradicated group, Conclusion The method is highly sensitive and specific, and is recommendable to apply for the diagnosis of Hp infection and the evaluation of Hp eradication.
