摘 要:目的研究生长相关蛋白-43(GAP-43)在膀胱过度活动症(OAB)患者膀胱表达及意义。方法应用免疫组织化学法和W estern b lot方法检测32例膀胱过度活动症患者膀胱组织及10例正常膀胱组织和10例其它原因所致尿频患者膀胱组织中GAP-43的表达,32例OAB患者中,13例为特发性逼尿肌不稳定(ID I),19例为特发性感觉急迫(SU)。结果膀胱过度活动症患者膀胱组织GAP-43阳性神经纤维的密度,较对照组膀胱组织高,二者间差异有统计学意义(P<0.05)。W estern b lotting结果显示,在NC膜上43 kDa处均有阳性的反应条带,正常膀胱和其他原因尿频膀胱反应带微弱;而OAB组中,ID I、SU组反应条带明显加深,蛋白平均积分光密度为0.6182±0.0891,0.5413±0.0823,二者间差异无统计学意义(P>0.05),而正常膀胱组为0.1828±0.0249,其它原因所致尿频为0.1957±0.0334,二者间差异无统计学意义(P>0.05)。OAB组与对照组间的差异有统计学意义(P<0.05)。结论GAP-43阳性神经纤维的密度增加,表明神经的再生和重新分布与OAB有关;GAP-43在OAB膀胱中的表达量增多,提示神经的发芽生长和再建重塑与膀胱过度活动症有关,可能参与OAB的发病机制。Objective To study the expression and clinical significance of growth associated protein 43 ( GAP-43 ) in the bladder of patients with overactive bladder (OAB), Methods The expression of GAP-43 was detected by Peroxidase immunohistoehemistry and western blotting in 32 cases of OAB, 10 cases of normal bladder tissue and 10 cases of patient with urge syndrome caused by other causes. Of 32 cases of patients with OAB, there were 13 cases of idiopathic detrusor instability (IDI) and 19 cases of SU, Results The expression of GAP-43 was significantly higher in bladders of patients with OAB compared with that of controls ( P 〈0. 05 ). The data of Western blotting indicated that the positive band was found around 43kDa on nitrocellulose membrane in all the cases. The band was slender in the controls and much deeper in cases of 1D1 and SU. The optical density of the 43kDa protein was 0. 6182 +0. 0891 and 0. 5413 +0. 0823 in the cases of 1D1 and SU respectively, and there was no significant difference between two groups ( P 〉0. 05). The optical density of the 43kDa protein were 0. 1828 +0. 0249 and 0. 1957 +0. 0334 in the cases of normal bladder tissue and urge syndrome caused by other cause respectively, without significant difference between two groups ( P 〉0. 05 ). There was significant difference in the optical density of the 43kDa protein between the patients with OAB and controls ( P 〈 0. 05 ). Conclusion The expression of GAP-43 is up - regulated in bladders of patients with OAB, which suggests that the neural sprouting and/or remodeling is involved in the pathogenesis of OAB.
