儿童肠旋转异常的影像学诊断（附27例报告）

摘　　要：目的提高对儿童肠旋转异常的影像学认识，着重讨论上消化道造影对本病的诊断价值。方法总结27例手术证实的儿童肠旋转异常影像学表现。27例均摄腹部平片．上消化道造影24例，钡剂灌肠10例，CT检查3例。结果肠未旋转3例，肠旋转不良24例。合并左膈疝、巨大脐膨出、环状胰腺、肠重复畸形、十二指肠降部膈膜、多处肠闭锁各1例。腹部平片为高位梗阻23例，其中17例为不全性，6例为完全性，1例小肠低位梗阻，左膈疝1例，肠道充气正常2例。24例上消化道造影均有十二指肠梗阻，不全性梗阻21例，其中9例有不同程度的中肠扭转，完全性3例。10例钡剂灌肠均见回盲部位置异常。结论上消化道造影对本病诊断有重要价值．可反映其基本病理改变，对疑有肠旋转异常的患儿应首选上消化道造影，必要时行钡剂灌肠检查。Objective To improve the imaging diagnosis of anomalies of intestinal rotation in children and to explore the diagnositic value of upper gastrointestinal barium. Methods 27 cases with anomalies of intestinal rotation in children proved by surgery were summed up. The diagnostic evaluations included 27 plain abdomen films, 24 upper gastrointestinal barium, 10 barium enemas and 3 CT. Results 3 cases of nonrotation and 24 malrotation was found, which companied with a case each of left hernia, large omphalocele, annular pancreas, duplication of the small bowel, diaphragm at the second part of duodenum and multiple bowel atresia. On the plain film high obstruction was showed in 23 cases including 17 incomplete and 6 complete onstruction. A low obstruction and left hernia were also found. Normal bowel air was showed in 2 cases. On upper gastrointestinal barium duodenum obstruction was found in 24 cases including 21 incomplete obstruction among which 9 showed midgut volvulus and 3 complete obstruction. Abnormal ileocecus position was found on barium enema in 10 cases. Conclusion Upper gastrointestinal barium which can reflecting basic pathology, has important diagnositic value to anomalies of intestinal rotation in children. The examination should be firstly selected to the children suspected anomalies of intestinal rotation and barium enema should be used when necessary.
