心内直视手术患者血浆中性粒细胞防御素的动态研究

摘　　要：目的定量观察心内直视手术患者围手术期血浆中性粒细胞肽(HNP 1-3)的动态变化,探讨在体外循环(CPB)诱发全身炎症反应综合征(S IRS)早期诊断中的作用。方法建立EL ISA法,对CPB患者围手术期不同时间血浆中HNP 1-3含量进行定量测定,同时测定血浆CK、CK-M B和C-RP含量。结果21例CPB心脏直视手术患者CPB前血浆HNP 1-3为(54.55±26.45)ng/m l;CPB后0.5h血浆HNP 1-3含量为(435.45±500.13)ng/m l,比CPB前显著升高(P<0.01)、CPB后4h血浆HNP 1-3含量达峰值,为(454.18±651.20)ng/m l,此后开始下降;血浆CK、CK-M B和C-RP峰值出现在CPB后24h,分别为(1 587.46±1 500.56)U/L、(38.18±39.27)U/L和(62.63±27.08)m g/L。结论血浆HNP 1-3对CPB诱发S IRS反应敏感、迅速,血浆浓度的动态变化可能提示S IRS的严重程度和发展趋势。Objective To observe the changes of human neutrophil peptide（HNP） 1-3 content in plasma quantitatively during perioperation of cardiac surgery under cardiopulmonary bypass （CPB）, and to explore the role of changes of HNP1-3 content in early diagnosis of system inflammatory response syndrome （SIRS） after CPB. Methods The contents of HNP1-3 in plasma during perioperation of 21 cardiac surgeries under CPB were measured qvantitatively using ELISA method, and the contents of CK, CK-MB and C RPinplasma were measured at the same lime points as that of HNP1-3. Results The contents of HNP1-3 in plasma were （54.55±26.45）ng/ml before CPB, and they were （435.45±500. 13）ng/ml 0.5h after CPB which were significantly higher than that before CPB （P〈0.01）. The contents uf HNP1-3 rose to peak value 4 hours after CPB, which were （454.18±651.20）ng/ml, and then the contents of HNP1-3decreased. The peak times ofCK, CK-MB and C-RP in plasma were 24 hours afterCPB, and the peak value;s of them were （1587.46±1590.56）U/L, （38. 18±39.27）U/L and （62.63± 27.08）mg/L respectively. Conclusion The response of HNP1-3 to the SIRS after CPB is sensitive and rapid, and the changes of content of HNP1-3 in plasma might be used to predict the seriot, s degree and tendency of SIRS after CPB.
