摘　　要：目的探讨干扰素(IFN-α)治疗慢性乙型肝炎(CHB)患者B淋巴细胞CD40的变化,为临床IFN-α治疗CHB提供一种观测指标。方法用免疫荧光双标技术流式细胞仪检测30例IFN-α治疗的CHB患者及12例对照组CHB患者治疗前、治疗后6个月B淋巴细胞CD40的百分率,同时检测IFN-γ的水平。结果对照组中,CD40I、FN-γ治疗前及治疗后无显著变化(P>0.05);IFN-α治疗组中,6个月后的CD40百分率低于治疗前(P<0.01),与对照组治疗6个月后比较,差异显著(P<0.05);IFN-γ高于治疗前(P<0.01),与对照组治疗6个月后比较,差异显著(P<0.05),CD40百分率与IFN-γ呈负相关(P<0.001)。结论CHB患者经IFN-α治疗后,体液免疫功能减弱,细胞免疫功能增强。B淋巴细胞CD40的监测可作为IFN-α治疗CHB的观察指标之一。Objective To explore the change of CD40 of peripheral blood B lymphocytes in patients with chronic hepatitis B（CHB） treated by interferon α（IFN-α）, and to provide an index in predicting the curative effect. Methods 30 patients with hepatitis B hospitalized were treated with IFN-α for 6 months and 12 patients of non-IFN-α treatment were controls, CD40 of peripheral blood B lymphocytes and IFN-7 were checked before treatment and in the 6th month of IFN-a treatment by using flow eytometry assay and ELISA respectively. Resalts The ratio of CD40 of the counts of B lymphocytes and the levels of IFN-T were not statistically significant in the control group（ P 〉 0.05） ; however, in the group treated with IFN-α treatment, the ratio of CD40 of the counts of B lymphocytes subsets were lower than those of before treatment （ P 〈 0.01）and those of controls treated （P 〈 0.05） ; IFN-γ were higher than those of before treatment（ P 〈 0.01 ）and those of controls treated （ P 〈 0.05）. The ratio of CD40 and the levels of IFN-γ had a trend of negative correlation （ P 〈 0.001 ）. Conehtsion Humoral immunity was weakened probably and cellular immunity was enhanced by IFN-α treatment in patients with CHB. CD40 can be as an index of IFN-α treatment in patients with CHB.
