老年2型糖尿病患者死因分析(附尸检报告)

摘　　要：目的明确老年2型糖尿病(T2DM)的并发症、合并症及主要致死原因。方法对近年来在我院死亡并进行尸检的老年T2DM患者72例的临床及尸检资料进行分析:(1)评估我院近年来住院死亡并进行尸检的T2DM的主要死亡原因,主要并发症与伴发病;(2)进入老年期后T2DM的发生率;(3)了解老年T2DM代谢控制情况。结果(1)循环系统疾病、恶性肿瘤、呼吸系统疾病占所有死亡病例的76.4%,与糖尿病直接相关的死亡占2.8%;(2)入院时患者血糖、血脂、血压等多种代谢指标控制未能达标;(3)进入老年阶段后新诊糖尿病占所有糖尿病病人的50%。结论老年新诊糖尿病占糖尿病的重要组成部分,与糖尿病直接相关的死亡原因所占比例较低,心血管疾病、恶性肿瘤、呼吸疾病是老年T2DM的主要死亡原因。Objective To understand the main causes of death and diabetic complications in senile diabetic inpatients. Methods The causes of death in 72 senile diabetic inpatients were investigated by antopsy. Results 1. The main causes of death were cardiovascular disease（36.1 %）, malignant tumor（20.8% ） and respiratory system disease（19.4% ） ,and the direct cause of death by diabetes was 2. 77%. 2. The metabolic abnormalities of glucose, lipids and blood pressure were controlled badly during hospitalization. 3. Half of the newly diagnosed diabetics were at old age. Conclusions Cardiovascular disease, malignant tumor and respiratory system disease are the main death causes in the senile inpatients. Newly diagnosed diabetes mellitus is the important part of senile diabetes.
