射频热凝损毁腰交感神经节治疗下肢缺血性疾病40例

摘　　要：目的探讨用射频热凝损毁腰交感神经节治疗下肢缺血性疾病的可行性。方法对40例因动脉缺血导致下肢疼痛和慢性溃疡者用热凝法损毁其腰交感神经节,观察患肢血氧饱和度(SPO_2)、趾脉搏波幅和足背皮肤温度,疼痛VAS评分变化以及肢体溃疡病灶的愈合情况。结果射频热凝损毁腰交感神经节有效改善了下肢末梢血液循环,缓解疼痛总有效率为90%；难治性溃疡病灶痊愈率达15%,明显好转占32．5%,好转占40%。结论射频热凝毁损腰交感神经节治疗下肢缺血性疾病疗效确实,定位精确、微创、成功率高、并发症少,是下肢缺血性疾病的有效治疗方法。Objective To evaluate the feasibility of radiofrequency ablation of lumbar sympathetic ganglion in treating the ischemia disease of the lower extremities. Methods Forty patients were treated by radiofrequency ablation of lumbar sympathetic ganglion with RFG-PLU3C instrument. The effect was analyzed by observing the alteration of SPO2 and skin temperature and ulcer healing in affected extremities as well as the VAS scores and the satisfactory degree of the patients. Results radiofrequency ablation of lumbar sympathetic ganglion expanded the affected artery, promoted the microcirculation, improved the nutrition condition of the extremities, ameliorated the pain and ulcer healing. The overall effective rate for pain was 90%. Ulcer was healed in 15%, significantly improved in 32. 5% and improved in 40% of patients. Conclusion radiofrequency ablation of lumbar sympathetic ganglion is effective for arterial ischemia disease of the lower extremities with accurate location, mini-injury, and few complications.
