PⅢNP与影像学检查在乳腺癌复发监测中的作用评价

摘　　要：目的：本文以影像学检查为参照，研究血清PⅢNP在乳腺癌患者复发监测中的应用价值。方法：选择成功随访36例术后复发转移乳腺癌患者为对象（其中肝转移21例，骨转移10例，肺转移5例）。乳腺癌病理分期：Ⅱ期8例、Ⅲ期28例。采用放射免疫分析乳腺癌患者术前、术后血清PⅢNP水平及CA15—3水平。36例乳腺癌患者术后均定期进行影像学检查。结果：血清PⅢNP预测复发早于影像学指标（6—9）个月，比CA15—3早3个月。结论：PⅢNP作为乳腺癌复发的熏要参考指标，术后PⅢNP水平较术前下降不明显或者持续升高，提示复发。血清PⅢNP预测复发较CA15—3及影像学早。Objective To compare the results of serum aminoterminal peptide of type Ⅲ procollagen （ PⅢNP） levels monitoring vs that with imaging studies for early detection of recurrence of breast carcinoma after surgical treatment. Methods Patients with breast cancer were examined for serum PⅢNP and CA15-3 levels measurement （with RIA） and imaging studies both before and every three months after operation up to 18 months. Elevation of the postoperative lowered tumor markers levels to above 60% of the preoperative values was designated as possible development of recurrence and the three - monthly examination was continued until definite recurrence was demonstrated with imaging studies. If possible, second operation was performed. Results Altogether, 36 patients were found to be with proven recurrences （ hepatic 26, osseous 10, pulmonary 5 ）. Elevation of the serum PIII NP level to above 60% of the pre - operative level usually took place 3 months earlier than that of CA15 - 3 level and 6 ～ 9 months earlier than the time of imaging study demonstration. Conclusion For early detection of post - operative recurrence of breast cancer, serum P ⅢNP level monitoring was more sensitive than CA15 -3 monitoring and imaging studies. A higher then expected level of serum PⅢNP post -operatively would warrant close observation.
