改良爱普沃斯困倦量表的信度、效度和相关因素研究

摘　　要：目的制定适合评价中国人群嗜睡状况的困倦量表。方法用较爱普沃斯困倦量表(ESS)增加两项的量表对临床鼾症组和对照组进行评分,经可靠性分析后得到改良爱普沃斯困倦量表(mESS)。评价mESS同质信度、半分信度和重测信度。用因子分析法评价其效度。用成组t检验比较OSAS各组、单纯鼾症组和对照组的mESS评分来评价其敏感性。结果增加两项的困倦量表cronbach’sα=0.85,将可靠性低的第七项剔除后得mESS。mESS,ESS的cronbach’sα分别为0.86和0.77。mESS同质信度最高。mESS半分信度rs=0.725(P<0.01)。重度OSAHS患者经CPAP治疗后mESS评分降低具有统计学意义(P<0.001),重测信度r=0.725(P<0.01)。因子分析得到2个公因子,其累计贡献率为0.5969。OSAHS组mESS评分与BMI指数、AHI指数,SpO2<90%时间所占比例显著正相关(r分别=0.415、0.468、0.392,P<0.01),而与性别,年龄无显著相关。结论mESS适合评价有临床鼾症的中国人群的嗜睡状况,并可作为我国OSAHS患者的初筛和OSAHS治疗后评价效果的方法。Objective To evaluate the reliability, validity and correlation analysis of the modified Epworth Sleepiness Scale （mESS）. Methods subjects were interviewed by using two - items added Epworth Sleepiness Scale. Reliability were estimated by cronbach＇s α, spit - half reliability and test - retest reliability. Validity of the scores was tested by factor analysis. Sensitivity was evaluate by comparison of the patients with sleep disordered breathing and controls. Results The cronbach＇s a of the two- items added Epworth Sleepiness Scale were 0.85 and 0.83. The mESS was got after the seven item was out. The cronbach＇s a of the scores of mESS were 0.87 and 0.84. The spit - half reliability were 0. 725 and 0.713 （P 〈 0.01）. The test- retest reliability of the 36 severe sleep apnea patients was 0.752 after treatment. Two factors were extracted by factor analysis. The scores of the mESS were not correlated with age and gender, but correlated with the BMI, AHI and the percentage of time spend at oxygen saturation〈 90% （ r=0.415,0.468,0.392, P〈0.01）in patients. Conclusion The reliability, validity and correlation analysis of the mESS were excellent. It was used as a evaluative tool in Chinese people and a primary diagnostic method in patients with snoring.
