摘 要:目的评价三维超声在诊断胎儿体表畸形中的临床应用价值。方法对常规二维超声检查怀疑有胎儿体表结构异常的病例进行三维超声检查,对获得的三维数据进行多平面分析及三维立体重建,将有体表畸形的15例胎儿的二维、三维超声检查结果与产后或引产后最后诊断结果进行分析,比较二维和三维超声检查判断病变部位及病变细节的准确性。结果三维超声扫查成功率为94.7%,检出胎儿体表畸形19处共10种畸形。二维及三维超声对病变部位判断正确者分别为78.9%(15/19)和94.7%(18/19),对病变细节判断正确者分别为63.2%(12/19)、89.5%(17/19)。结论三维超声图像立体、直观,不受体表复杂曲率变化的影响,对病变部位及细节能提供更丰富的诊断信息,较二维超声的诊断水平有明显提高,是二维超声的一种重要的补充形式。Objective The purpose of this paper was to investigate the dinical value of three-dlmensional ultrasonography (3D US) in diagnosis of fetal surface deformity. Methods The 15 pregnant women suspected of having fetal surface deformity with routine prenatal two-dimeusional ultrasonography (2D US) were examined with 3D US. Multi-plane analysis and three-dimensional reestablish of scanned volume data were used for diagnosis. The accuracy of 2D and 3D diagnosis for the malformation with detail was compared to the pathology outcome. Results The successful rate of 3D US scan was 94.4%. 18 parts of fetal surface deformity including 11 sorts of malformation were found in 15 eases. The accurate rate of prenatal 2D US and 3D US determined the site of anomaly were78.9%(IS/19)and 94.7%(18/19), respectively, and for described the abnormality in detail were 63.2%(12/19)and89.5%(17/19), respectively. Conclusions 3D US images is more tridimensinal and intuitionistic. It can provide more information in determining the site and describing the detail of fetal surface deformity than that of 2D US. So, 3D US can be used as an important complement method to 2D US.
