摘  要:目的:了解2005年天津医科大学总医院感染患者临床分离革兰阴性(G-)菌菌群分布及耐药性,为临床治疗提供实验依据。方法:按常规方法将患者血液、尿、粪便、分泌物等标本进行细菌培养,细菌鉴定和药敏试验采用法国Bio-Merieux公司的VITEK AMS-60全自动微生物系统GNI+细菌鉴定卡及GNS-506和GNS-135药敏卡片,部分抗生素采用kirby-Bauer法药敏试验。结果:共分离2 055株细菌,其中G-杆菌1 574株(76.59%),G+球菌481株(23.41%),分离率较高的细菌是铜绿假单胞菌株(14.26%)、大肠埃希菌株(14.26%)、肺炎克雷伯菌(13.38%)、醋酸钙鲍曼复合不动杆菌(7.45%)等。药敏试验显示,常见的革兰阴性杆菌(除嗜麦芽窄食单胞菌外)对美罗培南、亚胺培南、头孢哌酮/舒巴坦、丁胺卡那等抗菌药物敏感。结论:本院感染致病菌以G-杆菌占多数,G+球菌居次,这些致病菌对不同抗生素有一定程度的耐药性,临床医生在治疗感染性疾病时应参考药敏试验,合理应用抗生素。Objective: To investigate the distribution and drug resistance of G- bacilli isolated from the clinical specimen, and provide useful information for the clinical treatment. Methods: With routine processes, blood, urine, stool, excreta and other samples were cultured to isolate bacteria, isolation and identification of bacteria were conducted with VITEK-AMS 60 all-automatic microbiology analysis system and its bacilli identification cards (GNI+) and VITEK Susceptibility cards (GNS-506 and GNS-135). Disc diffusion test was partly used by K-B method. Results: 2 055 strains were isolated .Among them, 1 574 strains were of Gram-negative bacilli (76.59%). 481 strains of Gram-positive cocci (23.41%). The proportion of these strains from high to low were Pseudomonas aeruginosa 14.26%, Escherichia Coli 14.26%, Klebsiella pneumonia 13.38%,Acinetobacter baumann 7.45% etc. Almost all of Gram-negative bacilli were sensitive to meraopenem, imipenem, cefoperazone/sulbactam, amikacin etc. Conclusion: In our hospital ,Gram-negative bacilli are the major pathogenic bacteria, many of these organisms are resistant to different antibiotics to some extent, hence it is necessary for clinical physician to refer the drug sensitivity of bacteria so as to select antibiotics correctly.
