极化液严格控制血糖对严重烫伤大鼠炎症反应和体重的影响

摘　　要：目的探讨极化液严格控制血糖对严重烫伤大鼠感染炎症和体重的影响。方法30%的体表面积Ⅲ°烫伤大鼠80只,随机分为极化液组(n=20)、胰岛素组(n=20)、葡萄糖组(n=20)和盐水对照组(n=20)。极化液组和胰岛素组严格控制血糖到正常参考范围,观察不同组伤后1、3、5、7 d空腹血糖、血浆内毒素和肿瘤坏死因子α(TNFα)水平和2周后体重变化。结果极化液组和胰岛素组大鼠伤后各时相点空腹血糖、血浆内毒素和TNFα水平均明显低于盐水对照组(P均<0.01),葡萄糖组均明显高于盐水对照组(P均<0.01),极化液组和胰岛素组之间无显著差别(P均>0.05)。极化液组大鼠伤后2周体重明显高于盐水对照组(P<0.01),而胰岛素组和葡萄糖组与盐水对照组无显著差别(P>0.05)。结论科学使用极化液/胰岛素严格控制高血糖,可降低创伤后炎症反应。加大胰岛素剂量,补充葡萄糖可改善机体营养。Objective To investigate the effects of tight control of blood glucose by intensive GIK（Glucose-Insulin-Potassium） therapy on inflammatory response and body weight of severely thermal trauma. Methods 30% TBSA Ⅲ° scalded rats were randomly divided to GIK（n=20）, Insulin（n=20）,Glucose（n = 20） and control （71 = 20） groups. The fasting blood glucose in GIK group and insulin group were tightly controlled to normal levels. The levels of fasting glucose, LPS,TNFα and the body weight of rats in all groups were observed at 2 weeks after scalded. Results The levels of fasting blood glucose,LPS,TNFα of both GIK group and Insulin group were lower significantly than that of the Control group, Glucose group than that in control group. The body while they were were higher significantly in weight of scalded 2 weeks later of GIK group were higher significantly than that of the control group ,and there was no significant difference of body weight of scalded 2 weeks later between the insulin group, glucose group and control group. Conclusion Scientific treatment of GIK/insulin can contribute to reducing the inflammatory response and meliorating dystrophia enhanced by trauma.
