拉米夫定联合扶正解毒颗粒治疗慢性乙型病毒性肝炎临床观察

摘　　要：目的探讨拉米夫定联合扶正解毒颗粒口服对抗HBV及护肝作用的影响。方法根据肝功能有无异常分层,并按1∶1的比例随机分组。治疗组58例,口服拉米夫定和扶正解毒颗粒;对照组55例,口服拉米夫定,两组连续治疗1年。结果(1)治疗12个月时,治疗组的HBV-DNA阴转率较对照组高(P<0.05);治疗组治疗12个月的YMDD变异率较对照组低,但P>0.05;(2)治疗12个月时,治疗组的HBeAg阴转率较对照组高(P<0.05);治疗组的e系统的抗原抗体转换率较对照组高(P<0.05);(3)在肝功能异常的患者中,治疗组的复常率均较对照组高,但无显著差异。结论拉米夫定联合扶正解毒颗粒口服可以提高HBV-DNA、HBeAg的阴转率,提高HBe系统的抗原抗体转换率,促进肝功能恢复。Objective： The aim of this study was to evaluate the effect of Lamivudine combined with Strengthening Body and Detoxicating Decoction on patients with chronic hepatitis B. Method： 113 patients with chronic hepatitis B were randomized to combined therapy group （n= 58, using Lamivudine and Strengthening Body and Detoxicating Decoction） and control group （n= 55, using Lamivudine alone）. Both lasted for 1 year. Statistic process： z-test. Results：①The negative conversion rates （NCRs） of HBV-DNA in the combined therapy group were higher than those in the control group （each 3 months, P〈0.05 at the end of 1 year） ; The mutation rate of HBV-DNA in the combined therapy group was lower than that in the control group at the end of 1 year （but P〉0. 05）. ②The NCRs Of HBeAg in the combined therapy group were higher than those in the control group （each 3 months, P〈0. 05 at the end of 1 year）. The conversion rates of HBeAg to anti-HBe in the combined therapy group were higher than those in the control group （each 3 months, P〈0. 05 at the ends of 9, 12 month）. ③In the patients with abnormal ALT, the normalization rates of serum ALT in the combined therapy group were higher than those in the control group （each 3 months, but P〉0. 05 each）. Conclusion：These facts suggested that using Lamivudine combined with Strengthening Body and Detoxicating Decoction in the treatment was benefit to the patients with chronic hepatitis B.
