摘 要:目的:观察外源性锌对超长任意皮瓣成活的影响并探讨其机制。方法:96只大鼠随机分为正常组(n=32)、对照组(n=32)和处理组(n=32)。在大鼠背部设计蒂在头侧的任意皮瓣,约1cm×7cm大小。分别测定皮瓣组织中丙二醛(MDA)含量、髓过氧化物酶(MPO)活性和一氧化氮(NO)含量。观察免疫组化切片中金属硫蛋白(metallothioneinMT)的表达部位,并对切片进行图象分析,以积分光密度代表MT的含量。术后7天观察皮瓣成活率。结果:术后6h、12h,处理组MDA含量分别低于对照组15.7%、25.2%(P<0.05),MPO活性分别低于对照组21.5%、19.7%(P<0.05)。术后1h、6h、12h,处理组皮瓣NO含量高于对照组70.1%、109.8%、115.6%(P<0.01)。处理组中术后6h、12hMT表达量高于对照组47.7%、105.9%(P<0.01)。MT表达在皮瓣中表皮基底层细胞、皮下组织及肉膜下组织血管壁细胞、成纤维细胞,毛囊、皮脂腺、汗腺的细胞浆中。处理组皮瓣成活率较对照组有所升高。结论:外源性锌对超常任意皮瓣具有保护作用,可提高皮瓣的成活率。Objective To observe the influence of the exogenous zinc on the ultralong random flap and study the mechanisms. Methods A random skin flap(lcmx7cm) was created in Wistar rats. 96 rats were randomly divded into three groups: the normal group, the control group and the treated group. The content of malondialdehyde (MDA), the activity of myeloperoxidase (MPO) and the content of nitrogen monoxidum (NO) were measured. The location of expression of MT was observed, and the image analysis was performed. The quantity of MT was represented by the integratial optical density. 7 days after the operation the flap viability was observed. Results Compared with the control group, 6h and 12h after the operation, the level of MDA in the treated group decreased 15.7%,25.2%(P〈0.05), the activity of MPO decreased 21.5%,19.7%(P〈0.05), the quantity of MT increased 47.7%, 105.9%(P〈0.01), respectively. Compared with the control group, lh, 6h and12h after the operation, the content of NO increased 70.1%,109.8%,115.6%(P〈0.01). In the treated flaps, MT was located in cytoplasm of epidermal basal cells, the vascular endothelial cells in subcutaneous tissue , fibroblasts, hair follicle, sebaceous glands and sweat glands. Compared with control group, the flap viability of treated group increased. Conclusion The exogenous zinc had the protective effects on the ultralong random flap. The random flap viability increased.
