摘 要:目的了解武汉市麻疹暴发原因,提高麻疹监测系统敏感性。方法对2004年10月至2005年9月麻疹监测数据进行分析。结果2004年10月至2005年9月全市麻疹监测系统共报告麻疹疑似病例1 944例,其中诊断麻疹1 827例,发病率为23.25/10万。麻疹发病呈散发和暴发并存模式,发病以<15岁儿童为主。在确诊病例中有麻疹疫苗(M V)免疫史占23.16%,无免疫史占41.94%,免疫史不详者占34.90%。暴发主要原因是部分地区M V常规免疫不落实、传染源管理不善。结论加速麻疹控制应采取以加强常规免疫和强化免疫为主的免疫策略,同时提高监测系统的敏感性和质量,加强疫情报告和管理,预防麻疹暴发。Objective To prevent and strengthen the control of measles prevalence in Wuhan and improve the sensitivity of measles surveillance system. Methods To analyze the data of the measles from Oct. 2004 to Sep. 2005. Results 1 944 measles cases were reported and the number of diagnosed cases was 1 827. The incidence rate of measles was 23. 25 per 100 000. Sporadic and outbreak cases were all included. Cases of lower than 15 years old were higher than the other groups. Among all the diagnosed cases, the rate of immune cases occupied 23. 16%, Non-immune cases were 41.94% and the immunity history unknown cases amounted to 34.90%. The main reason was that the rountine measles vaccination (MV) was not well implemented and the source of infection was not properly managed. Conclusion The strategy of enhancing the rountine immunization and booster immunization of MV should be adopted the sensitivity and quality of surveillance system should be improved, and the reporting system should be reinforced so as to prevent the outbreak of measles.
