留置中心静脉导管治疗恶性胸腔积液的护理

摘　　要：目的总结留置中心静脉导管引流恶性胸积液及胸腔内化疗的护理要点。方法对23例肺癌合并胸腔积液患者在B超定位下经皮穿刺胸腔内留置中心静脉导管,间断抽吸或引流胸腔积液后注入化疗药物,并做好导管留置期间护理。结果23例均能成功置管,顺利完成治疗,置管时间最长32 d,最短7 d,平均16.5 d,无1例发生感染等严重并发症。结论护理上应严密观察病情变化,协助患者更换体位,严格执行无菌技术,预防感染,保持导管固定通畅,防止导管脱落是治疗成功的关键。Objective To observe the effect of indwelling central venous catheter to drain the malignant hydrothorax and the effect of innerthoracic chemotherapy, and summarize the nursing points. Methods Central venous catheters were indwelled inside the chest of 23 patients with lung cancer by eentesis under B ultrasound orientation. Results All catheters were indwelled successfully and the treatment was finished without any serious complication. The longest catheter indwelling time was 32 days while the shortest time is 7 days, average time being 16. 5 days. Conclusion To ensure successful treatment, nurses should observe patients closely, help patients to change positions, carry out the aseptic technique stricdy, prevent infection and keep the catheter clear and fixed well.
