关节镜下诊治Hoffa氏病60例临床分析

摘　　要：目的 探讨关节镜诊治Hoffa氏病的疗效。方法 2000年1月～2005年1月收治60例膝关节Hoffa氏病患者，均行关节镜下髌下脂肪垫部分切除术。对合并半月板损伤的12例行半月板部分切除；滑膜皱襞综合征的8例切除滑膜皱襞取出游离体；ACL损伤的3例中2例行重建术，1例行关节清理术。术后早期功能锻炼。结果 平均随访18.5（6-46）月，疗效评价：优45例，良7例。差8例，优良率86，7％。结论 关节镜下行髌下脂肪垫部分切除术治疗Hoffa氏病具有创伤小、康复快、疗效可靠的优点。Objective To study the advantages and reliability of arthroscopic therapy for Hoffa＇s disease. Methods 60 patients of Hoffa＇s disease admitted from January 2000 to January 2005 underwent partial excision of infrapatellar fat pad by arthroscope. 12 cases of meniscus injury underwent partial meniscus removal, 8 cases of synovial plica syndrome underwent removal of corpus liberum. Joint debridment and reconstruction were carried out in 1 and 2 cases of anterior cruciate ligament injury respectively. All cases received early stage dirigation. Result The outcome were excellent in 45 cases, good in 7 cases and poor in 8 cases in consistent with 6 to 46 months follow-up. Conclusion Partial excision of infrapatellar fat pad under arthroscope is effective and minimally invasive for Hoffa＇s disease.
