摘  要:目的运用多普勒超声技术,探讨高血压左心室局部心肌松弛不同步性对舒张早期血流传播速度(VP)的影响。方法原发性高血压患者23例和正常人25例,舌下含服硝苯地平10mg,应用组织多普勒和彩色M-型多普勒测量用药前后左心室心肌松弛不同步性指数(AI)和VP。结果服药前,高血压组较对照组AI增大、VP减小(P均<0.01),AI和平均动脉压是VP独立性影响因素。服药后,高血压组AI减小、VP增大(P均<0.01),VP与AI的变化率高度相关(r=-0.74,P<0.01)。结论左心室心肌松弛不同步性是高血压VP主要决定因素,硝苯地平通过改善心肌松弛不同步性而使VP增大。Objective To investigate the effects of regional left ventricular relaxation asynchrony on early diastolic flow propagation velocity (Vp) in hypertension. Methods In 23 patients with essential hypertension and 25 normal controls ,regional left ventricular relaxation asynchrony index (AI) and Vpwere measured using tissue Doppler imaging and color M-mode Doppler before and after nifedipine was administered sublingually. Results AI was larger in the hypertensive group than in the control group (P〈0. 01) while Vp was lower in the hypertensive group (P〈 0. 01) .AI and mean blood pressure were two independent determinants of Vp in the hypertensive group before administration of nifedipine. After administration of nifedipine in the hypertensive group, AI was decreased (P〈 0. 01 ), Vp was increased (P〈0. 01), while the percentage change in Vp correlated strongly with that in AI (r= -0. 74,P〈0. 01) . Conclusions Regional left ventricular relaxation asynchrony is a major determinant of Vp in hypertensive patients. Reduced regional left ventricular relaxation asynchrony after the administration of nifedipine contributed to increase Vp in hypertensive patients.
