摘 要:目的:探讨郎格罕氏组织细胞增生症(LCH)的超微病理特点,提供Birbeck颗粒来源的线索。方法:结合光镜、免疫组化及临床资料,对2例LCH进行了超微结构观察。结果:朗格罕组织细胞胞浆内可见棒状、网球拍状Birbeck颗粒,这些细胞免疫组化CD1a阳性。结论:LCH的诊断须见到典型的郎格罕氏细胞(Langerhans cell,LC)LC细胞,电镜下找到Birbeck颗粒,或CD1a(O10)阳性。Birbeck颗粒可与细胞膜相连续,细胞膜凹陷可在胞质内形成Birbeck颗粒。Objective: To study ultrastructural features of Langerhans-cell histiocytosis(LCH) and provide some ultrastructural evidence to clarify the origin of Birbeck granules. Method: 2 cases of LCH were selected randomly. Specimens were treated with routine LM and TEM procedures, immunohistochemical method, and examined by light and electron microscope. Results: The Langerhans' cells showed positive immunostain for CDIa and S-100 protein. Under the electron microscope, ultrastructure of the Langerhans' cells revealed typical intracytoplasmic tennnis racquet-shaped structure with a central zipper-like striation (Birbeck's granules). Conclusion: Birbeck's granules can serve as a useful marker for the identification of Langerhans-cell histiocytosis. The CDla and S-100 protein are demonstrated by immunohistochemical method, can serve this purpose as well. The development of Birbeck's Langerhans-cell granules is in evidence by invagination of the cell membrane ultrastructually.
