脂血康对冠心病患者血浆HDL亚组分水平及颗粒直径的影响

摘　　要：研究了中药脂血康煎剂对冠心病１０２例的血浆脂蛋白、载脂蛋白（Ａｐｏ）、高密度脂蛋白（ＨＤＬ）亚组分ＨＤＬ２、ＨＤＬ３水平的影响，并通过４％～３０％聚丙烯酰胺梯度凝胶电泳观察了给药前后ＨＤＬ亚组分的颗粒直径和相对百分含量的变化。结果显示，脂血康能显著降低患者血浆甘油三酯（ＴＧ，Ｐ＜０．０５），总胆固醇（Ｔｃ，Ｐ＜０．０５），低密度脂蛋白胆固醇（ＬＤＬｃ，Ｐ＜０．０５）和ＡｐｏＢ（Ｐ＜０．０２）的水平以及冠脉风险指数（ＣＩ，Ｐ＜０．００５），升高ＡｐｏＡ－Ⅰ（Ｐ＜０．００５）、ＨＤＬｃ（Ｐ＜０．０１）和ＨＤＬ２ｃ（Ｐ＜０．００５）的水平，增加颗粒直径较大的ＨＤＬ２ｂ的相对百分含量（Ｐ＜０．０５），而且作用优于复方丹参片。提示脂血康能有效地改善冠心病患者的脂蛋白代谢，影响ＨＤＬ的化学组成和颗粒分布，促进胆固醇逆向转运，从而预防动脉粥样硬化病变的发展。One hundred and two patients with coronary heart disease(CHD) were treated by ZXK in 8 weeks, the levels of plasma lipoproteins, apolipoproteins and HDL subfractions HDL2cand HDL3c were measured , the particles size of HDL subclasses and the ratio of each in whole HDL was analysed by 4% to 30% polyacrylamide gradient gel electrophoresis. It has shown that the concentrations of TG (P<0. 05 ) , Tc (P<0. 05 ), LDLc (P<0. 05)and ApoB(P<0. 02)were lower, while the levels of ApoA-I (P<0. 005 ) ,HDLc(P<0. 01 ), HDL2c(P<0. 005 ) were higher significantly after ZXK administration than before. The ratio of larger particle subclass HDL2b increased (P<0. 05). The ZXK can improve CHD patients' lipoproteins metabolism, alter HDL composition and subclasses distribution to increase the ability of reverse cholesterol transport and prevent the development of atherosclerotic lesions.
