摘　　要：目的研究不明原因流产患者血清蛋白质谱特征,探讨不明原因自发性流产的可能病因。方法采用表面增强激光解离飞行时间质谱技术(SELDI-TOFMS),运用WCX2蛋白质芯片检测39例不明原因流产患者和30例正常早孕血清蛋白质谱,获得的蛋白质谱用Biomarker Wizard软件分析,初步筛选蛋白质峰,结合生物信息学的支持向量机方法建立并测试不明原因流产患者血清蛋白质指纹图谱模型。结果在芯片上捕获到251种蛋白质,经蛋白质质谱仪分析发现不明原因流产患者与正常对照组有29种差异蛋白,从中筛出差异显著的5种蛋白质组成原发不孕患者血清的蛋白质谱最佳优化模型,其中质荷比(m／z)分别为8 350,6 954,8 634,8 826的4种蛋白质表达下降,质荷比为(m／z)6 628的蛋白质表达上调。模型经三倍交叉验证实验测定,其敏感性为83．9％,特异性为83.7％。阳性预测值为81．1％。结论不明原因流产患者与正常早孕者血清蛋白质谱存在明显差异,深入研究可能为不明原因流产的病因学提供基础,同时有助于自发性流产血清生物标记物的筛查和发现。Objective To characterize serum proteomic spectra in patients with unexplained spontaneous abortion and to explore possible cases of spontaneous abortion. Methods Proteomic spectra of serum samples from 39 patients with unexplained spontaneous abortion and 30 healthy pregnant women were analyzed by surface-enhanced laser desorption/ionization time of flight mass spectrometry （SELDI-TOFMS）. The peaks were detected by WCX2 （weak cation exchange） proteinchip array first. With support vector machines （SVM） method of biologic informatics, a system of serum protein finger printing was developed for detecting the abortion patients. Results 29 differentially expressed proteins in serum were discovered by analysis of proteomic spectra in 251 proteins captured by the proteinchip array. 5 proteins, which showed differences between the two groups, were obtained and made up the markers pattern of infertility patients, of which, 4 protein （8 350, 6 954, 8 634, 8 826 m/z） expressions were down-regulated and one （6 628 m/z） was up-regulated. Through cross-validation test, the sensitivity is 83.9 %, specificity is 83.7 %, and the positive predict value is 81.1%. Conclusion There are prominently differences in serum proteomic spectra between patients with unexplained spontaneous abortion and healthy pregnant women.
