川芎嗪预防术后肠粘连的实验研究与临床应用

摘　　要：目的观察川芎嗪腹腔灌注对术后肠粘连的预防作用。方法建立家兔肠粘连模型,分为对照组、川芎嗪组、糜蛋白酶组和川芎嗪+糜蛋白酶组,采用术中腹腔灌注药物,术后15 d处死动物评价效果。在动物实验的基础上将川芎嗪应用于临床70例腹部手术病人观察临床疗效。结果川芎嗪组有明显的预防术后肠粘连作用,其效果优于糜蛋白酶组。临床观察,川芎嗪治疗组肠鸣音恢复时间、首次排气排便时间明显早于对照组,远期随访无粘率及总体疗效亦优于对照组。结论川芎嗪腹腔灌注对术后肠粘连有明显的预防作用,值得进一步推广应用。Objective To observe the efficacy of Ligustrazine administered through intraperitoneal perfusion during operation in preventing postoperative instestinal adhesion. Methods Sevnty - two New Zealand rabbits were made to intestinal adhesion model and divided into 4 groups,the group A（ the control group）, B （treated by Ligustrazine ）, C（treated by Chymotrypsin）, D （treated by Ligustrazine ＋ Chymotrypsin ）. The model animals were sacrificed on the 15th day after operation and their intestinal adhesion were determined separately. Basing on the experimental study, the clinical therapeutic effect of 70 cases with abdominal operations treated by Ligustrazine were observed. Results Animal experiment showed that Ligustrazine has obvious effect of intestinal adhesion prevention , and its effect was evidently superior to Chymotrypsin. Clinical observation showed that the recovery of borborygmus,the first passing of flatus -defecation were much earlier in the group treated by Ligustrazine than those in the control group. Fol- low- up study also showed the treated group was better than the control group in the non -adhesion rate and the total effective rate. Conclusion Ligustrazine administered through intraperitoneal perfusion during operation has good effect on preventing the occurrence of postoperational intestinal adhesion, It has a good prospect of development and application.
