摘 要:观察囊型肝包虫囊周肝细胞的病理形态学变化(肝细胞萎缩、坏死、凋亡),初步探讨囊型肝包虫病肝细胞“消失”机制。对30例肝包虫囊肿周围肝组织及10例正常肝组织通过光镜观察肝细胞的病理形态学变化,透射电镜观察6例肝包虫囊肿周围肝细胞超微结构改变,运用TUNEL法测定肝细胞凋亡并采用免疫组化技术检测30例肝包虫囊肿周围肝组织及10例正常肝组织中Bcl-2及Bax蛋白的表达。结果显示TUNEL法测定囊周肝细胞凋亡发生极少(0.12%)与正常肝(0.16%)比较无显著差异,囊周肝组织Bcl-2及Bax蛋白呈低表达,分别为6.6%和13.3%,病理学观察囊周肝细胞明显萎缩,肝细胞坏死。因此,肝细胞萎缩、坏死、凋亡共同参与囊型肝包虫囊周肝细胞“消失”,肝细胞萎缩、坏死可能是引起肝细胞“消失”的主要机制,肝细胞凋亡可能不是引起肝细胞“消失”的主要机制。To observe the pathologic changes (atrophy, necrosis, apoptosis )of pericystic hepatocytes in hepatic hydatid disease, Preliminary study the mechanism of hepatocyte "disparence" on hepatic hydatid disease was carried out using light microscope and TDT-mediated Desoxyuridine Triphosphate nick end-labeling(TUNEL), meanwhile, the expressions of Bcl-2, Bax protein were detected by immunohistochemical method in 30 cases of the pericystic tissues of hepatic hydatid, 6 cases were sent to TEM laboratory for observing ultrastructure of hepatocytes, 10 cases of normal hepatic tissues were assigned to controlled group. The results of TUNEL assay show that AI of hepatocytes apoptosis is 0.12% in the pericystic hepatic tissues,the expression of Bcl-2 and Bax protein were 6.6% and 13.3% respectively; the pathological obversation indicate that hepatocyte atrophy and necrosis can be seen obviously. The results indicate that It is comprehensive mechanisms to induce the hepatocyte "disparence", hepatocyte atrophy and necrosis are possibly a predominant mechanisms to hepatocyte "disparence"in hepatic hydatid disease,hepatocyte apoptosis maybe is minor.
