冠状动脉钙化及其程度与冠心病危险因素关系的研究

摘　　要：目的 研究冠状动脉钙化及其程度与糖尿病、高血压、高血脂等冠心病危险因素的关系。方法 2004年3～12月来我院就诊及体检者共538例，将其分成冠心病组（67例）、疑似冠心病组（336例）和非冠心病组（135例）。采用问卷调查的方法分析冠状动脉钙化率、钙化积分与冠心病及其危险因素的关系。结果冠心病组冠状动脉钙化率和钙化积分均高于非冠心病组，疑似冠心病组处于二者之间，冠状动脉钙化积分随冠心病患病时间的延长而不断增加；在非冠心病组非条件Logistic多元回归模型分显示糖尿病和高血压是冠状动脉钙化的危险因素（OR的95％CI为3．015～41．691和1．899～11．617）；多元线性回归模型分析显示糖尿病、高血压及其患病时间为冠状动脉钙化积分的危险因素（P〈0．05）。结论 冠状动脉钙化与冠心病有联系；性别、年龄、高血压、糖尿病等因素影响冠状动脉钙化：冠状动脉钙化与冠心病的危险因素同时存在时提示有促进亚临床冠心病发展的作用。Objective To study the correlation between coronary artery calcification scores and risk factors of coronary heart disease such as diabetes, hypertension, high blood lipid and so on. Methods This study was carried out in 538 cases including body - check persons in our hospital from March to December in 2004. The correlation of coronary artery calcification （CAC） scores and risk factors of coronary heart disease （CHD） were analyzed by questionnaire. Results CAC incidence and CAC scores were significantly different in the 3 groups （ no CHD group, doubtful CHD group, CHD group）, CAC scores increased following the prolonging of sicken time. In the group without CHD, the logistic analysis of multi - factors showed the risk factors of CAC incidence were diabetes and hypertension （ 0R95 % CI = 3.015 - 41.691 ; 1. 899 - 11.617 ）. The multi - line regress showed the risk factors of CAC scores were diabetes, hypertension and sicken time （ t = 1. 987, 6. 472 ; P 〈 0.05 ）. Conclusion There is evident relation between CHD and CAC. The risk factors of CAC are sex, age, hypertension, diabetes and so on. CAC existing with the dangerous factors of CHD indicates that the risk factors play a role in promoting the development of sub - clinic CHD.
