摘  要:目的采用神经导航辅助脑室镜技术手术治疗儿童侧脑室蛛网膜囊肿,探讨两者结合应用对开展小儿神经内镜手术的意义。方法对7例患儿,男5例,女2例,运用神经导航制定手术计划,设计理想的手术切口和入路,实时、动态地监测内镜的操作轨迹。内镜下行囊肿开窗术4例,切除术3例。结果全部病例均获得良好的治疗效果,切除手术中次全切除2例,部分切除1例。术后无菌性脑膜炎2例,无出血、感染及神经功能缺失等并发症。结论结合神经导航技术,可以弥补病理情况下脑室内局部解剖变异导致内镜操作中定位困难的缺陷,使脑室内复杂手术的精确性更高,手术安全有效。神经导航辅助脑室镜技术在儿童微创神经外科手术中具有广阔的发展前景。Objective To evaluate the results of neuronavigation-assisted ventriculoscopy technique in the treatment of intraventricular arachnoid cysts in children.Methods Under the guidance of neuronavigation,surgical plan including optimal skin incision and approach was conducted.The trajectory of ventriculoscopy was monitored and guided in real time and dynamically by neuronavigation system.Cyst enestration or resction was performed in 3 cases and 2 cases ,respectively.Results After operation,all patients got the satisfactory results.There was no mortality and the serious complications such as hemorrbage.CNS infection and focal neurological deficits.Aseptic meningitis was noted in 1 patient who presented with temporary fever,Conclusions The combination of ventriculoscopy and neuronavigation is an accurate,effective and safe procedure for children with intraventricular arachnoid cysts.
