声学密度技术量化病毒性心肌炎心肌损害的研究

摘  要:目的:探讨病毒性心肌炎心室壁运动异常节段的组织声学密度(AD)特征及临床意义。方法:2000年1月至2004年8月,应用声学密度定量(AD)技术分析30例健康儿童左室壁各节段,31例病毒性心肌炎患儿左室壁运动异常节段和无异常节段不同时期的平均图像强度(AII)和周期性变化幅度(PPI)值,部分患儿做了单光子发射计算器断层摄片术(SPECT)心肌核素显像检查,结果与组织多普勒成像(DTI)进行对照。结果:30例正常儿童心室壁AD值随心动周期呈现规律性变化,比较左室各壁同一节段间隔数天的AII值和PPI值均无显著性差异;31例病毒性心肌炎患儿室壁运动减弱节段在急性期异常状态时AD值与恢复期正常状态后AD值对比,异常状态下AII值显著高于恢复状态的AII值(T=3.12,P<0.05),PPI值无显著差异(T=1.12,P>0.05),而急性期室壁运动无异常的节段不同时期对比,AII值与PPI值均无显著性差异;SPECT心肌显像所见室壁异常区域与DTI室壁运动异常区域具有高度相关性(χ2c=43.3,P<0.01)。结论:病毒性心肌炎患儿组织多普勒显示室壁运动异常节段的AD特征与运动状态恢复后的显著不同,结合组织多普勒检查,AD动态分析对炎性心肌具有组织识别和转归评价作用。Objective: To explore the value of acoustic density(AD) characterization in the recognition of inflammation myocardium in viral myocarditis. Methods. Thirty healthy children and 31 children with viral myocarditis were studied with AD technique from January 2000 to august 2004, all the patients met the diagnostic criteria of national viral myocarditis and had regional ventricular wall abnormality(RVWM) determined by Doppler tissue imaging(DTI), and 18 cases of all the patients examined with Single Photon Emitting Computerized Tomography(SPECT). The measurement parameters of AD included average imaging intensity(AII), peak to peak intensity(PPI) and standard deviation imaging(SD). Results: The AD value of myocardium varied cyclically accord- ing with cardial cycle, which was maximum in end-diastolic period and minimum in end-systolic period. AII of RVWM area was significantly higher in the acute stage of myocarditis than that in the recovery stage(T=3.12, P〈0. 05), while no significant difference in PPI was found (T= 1.12, P〉0.05). Conclusion: AD characterization of inflamed myocardial area is far different from healthy myocardium, therefore. It have significant effect of identifying abnormal myocardium in viral myoarditis.
