带蒂大网膜填塞囊腔术治疗成人多囊肾

摘　　要：目的提高成人多囊肾的手术疗效。方法对21例多囊肾患者实施囊肿2～3级的“囊中囊”去顶减压后，根据网膜解剖特点裁剪大网膜，分束填塞至囊肿底部。结果术后1个月，患者主观症状均明显改善，血压下降，肾血流增加，肾功能改善。术后5年监测：①囊肾再次增大，超过术前水平者16．7％（2／12）；②有75％（9／12）的患者症状较术后1个月无加重；③肾功能较术前无明显恶化者100％（12／12）；④血压有所升高，但无统计学意义。结论囊肿去顶减压后用带蒂大网膜填塞囊腔是治疗成人多囊肾的一种较好的新方法。Objective To improve the operative treatment of autosomal dominant polycystic kidney disease （ADPKD）. Methods Depression plus pedicled omental tamping was carried out in 21 patients with ADPKD. Results Success was achieved in all of the patients. Five- year follow- up of 12 cases showed recurrence of the renal cyst in 2 cases （ 16.7% ）, sustained symptomatic improvement 1 month after operation in most of them （75%）, no deterioration of renal function, and insignificant elevation of blood pressure in some cases. Conclusion Depression and pedicled omental tamping is a new good treat- ment for ADPKD.
